A licensed skilled nursing facility (SNF) that provides Short Term Rehab (physical, occupational & speech therapy) and long-term care services for up to 50 residents at Forest Farm Assisted Living. Forest Farm is staffed on a 24-hour basis with qualified nurses and certified nursing assistants. The services of the medical director, assistant administrator, a registered dietician, a pharmacist's consultant, and a social worker are also available. Two components: Short term rehabilitation after hospital, or long term care both at Forest Farm Assisted Living.
