Eligibility
-The individual must be a U.S. citizen or be a permanent resident and have lived in the U.S. continuously for five years.
-Must be 65 or older or be between the ages of 18 - 64 and receiving Social Security -Disability Insurance (SSDI) for at least 24 months or receiving Social Security Disability Insurance (SSDI) because of a diagnosis of ALS (Lou Gehrig’s disease) or have End-Stage Renal Disease (ESRD).
