Eligibility

-The individual must be a U.S. citizen or be a permanent resident and have lived in the U.S. continuously for five years. -Must be 65 or older or be between the ages of 18 - 64 and receiving Social Security -Disability Insurance (SSDI) for at least 24 months or receiving Social Security Disability Insurance (SSDI) because of a diagnosis of ALS (Lou Gehrig’s disease) or have End-Stage Renal Disease (ESRD).

The Senior and Disabilities Services at United Way, also known as the POINT, is a statewide resource network that provides information, referrals, and long-term care options to seniors, adults with disabilities, and their caregivers. Get valuable information and assistance about: - Information and assistance regarding home and community care - State Health Insurance Program (SHIP) - Public assistance programs such as Medicare, Medicaid, SNAP, LIHEAP, and others. - Information regarding other valuable resources in the community such as healthcare fraud and home care This service is free and confidential.
