New Horizons Adult Day Center, part of VNA of Care New England, provides structured programs during the day in a community group setting to support the personal independence of older adults and those that are disabled or have cognitive decline. Programs are designed to promote the physical, social, and emotional well-being of participants while providing health care and food services. Services include: Respite for Caregivers, Support for those with Cognitive Decline, and Day Group and Individual Support Services (physical, occupational, speech needs, case management and nursing healthcare services). Meal Program - Breakfast, lunch and snack are served daily. Meals are catered and meet USDA requirements. Medication Management - behavioral management and monitoring, blood sugar testing, diet management, health education, medication administration, monthly status assessments, personal care and incontinence assistance and vital sign checks. Therapeutic Recreation & Activities - arts & crafts, dance/movement, entertainers, exercise, games, movies, music, special interest groups (baking, cards, gardening, knitting, walking), sensory stimulation & time orientation. Case Management - applications for state financial aid, home care service coordination, care plan meeting facilitation, long term care planning and counseling for caregivers and family members.
