Must be between 2.5 and 18 at time of referral and have a life threatening medical condition that can be verified by their physician. Child cannot have received a wish from any other wish granting organization.

Child must be referred by a medical professional, a potential wish kid, a parent/legal guardian or family member of the potential wish kid with detailed knowledge of the child's current medical condition. Complete online referral inquiry form or contact your local chapter to begin the application process.
