Eligibility

Be age of 65 years or older, or individuals with disabilities who are over 18 years old Possess the ability to self-direct all aspects of their personal care and community living needs, or have a representative who is available and willing to direct and manage their care and community living needs Have a cognitive or physical disability that affects their ability to complete Activities of Daily Living in a safe or timely manner Be eligible for Medical Assistance through The Rhode Island Department of Human Services Long Term Care

Description

Age Details: Child development: 3 - 21 years; sheltered workshops & residential: 21 years and over Provides children and adults with developmental disabilities the means to lead full and productive lives. Adult Services Include: Vocational and Day Services Adult Day and Community Based Services Employment Services Residential Services Senior Services Program * Comstock Industries provides on-site adult employment opportunities * Children's Services: The Cornerstone School and Transition Services * Home-Based Treatment Income Source: United Way; State; Federal; City; contributions. Personal Choice Program: You may hire and manage your own caregiver who can help you with bathing, dressing and other home activities. You will have a budget to cover the costs of your caregiver's pay and the other services you choose to receive through this program.
