Description

Children’s Personal Assistance Services and Support (PASS): consumer-directed program designed to support children with special health care needs. PASS provides families with the opportunity to take the lead role in directing the services received by their child. The family-centered approach empowers families and individuals to foster independence, support family care giving and decision making, respect family choices and values, and build on individual strengths. Respite Program: a consumer-driven program designed to enhance the quality of life for children with special needs and their families. The provision of Respite enables families to receive support in the delivery of care for their children. Children’s Home-Based Therapeutic Services (HBTS): clinically directed, specialized health services that are designed to meet the individual needs of children diagnosed with moderate to severe physical, developmental, behavioral or emotional conditions. Addresses behavioral concerns and the development of functional, social and communication skills. The HBTS Team is comprised of the parent(s), a clinical supervisor, a home based therapist, a family service coordinator, and when indicated specialty (treatment consultants, speech therapy, physical therapy, and/or occupational therapy). Together, the HBTS Team develops an individualized treatment plan with measurable goals and objectives. Additionally, clinical support and training is provided on a continuous basis to the home based therapist and family.
