Description

Home-Care, in affiliation with South County Hospital, for adult patients under the care of a licensed physician, with acute, chronic, or terminal illnesses. Services available to patients and their families include skilled nursing, physical therapy, speech therapy, occupational therapy, social work, wound care specialist, Certified Nursing Assistant and nutrition consultation. Community Programs Health Clinics for senior citizens, home nursing visits to families with newborns. JCAHO Accredited. HomMed Home Telemonitoring: Technology that monitors vital signs daily while at home and automatically transmits them to the South County Home Health office where a registered nurse reviews them. First Connections services high risk moms and their children from birth to age three. Families are identified through the birthing hospitals as high risk and in need of community support. The intent is to reach all new moms within seven days after delivery or within 24 hours if the mom is “High Priority”Pregnant women. Healthy Families America is a national evidence-based maternal and child home visiting service. HFA pairs expectant mothers or families with newborns with a Family Support Worker, who provides free home visiting services to the family and their child until the child’s fourth birthday.
