Description
Solutions is a program of Family Service of Rhode Island that provides care coordination across a continuum of service providers involved with children and their families. This continuum includes medical, educational, mental health and other providers. In addition to care coordination, Solutions services include a global assessment of need, an action plan, assistance with referrals and linkages while providing ongoing support to ensure the best possible health and well-being outcomes.
