The RI Department of Health and Human Services and the RI Chapter of American Pediatrics are collaborating on a Medical Home pilot project to assist pediatric and family practices that provide primary care for children with special health care needs (CSHCN). Parent Consultants assist physicians and medical staff to provide a comprehensive coordinated medical home to hundreds of families. PC's provide families with real-education and navigation of other complex service systems that provide serives and support to families who have (CSHCN).
