Elder Case Management: Provides assessment, monitoring, and protective services for seniors. Case managers oversee the development and implementation of client-specific care plans. They can help clients maintain independence through home and community services. These services are state-subsidized through the RI Division of Elderly Affairs’ Home and Community Care Programs. In general, the Elder Case Management Unit helps residents of Kent County who are at least 60 years old or in assisted living. Programs and Services Assessment - Case managers perform a general needs assessment to develop an effective plan of care. Protective Services - Case managers provide supportive services for at-risk older residents referred through the Division of Elderly Affairs. Co-Pay - Eligible home-bound clients aged 65 and older pay a reduced rate for the services of a certified nursing assistant. Waiver Program - Medicaid-eligible clients receive the services of a certified nursing assistant at home. The cost is based on income. Assisted Living Case Management - Medicaid-eligible clients receive care in local assisted living facilities at an income-based cost. RSVP (retired Senior Volunteer Program): Services: Training and placement of senior citizens in volunteer assignments which match their interests and skills. Encourages seniors to remain active in the community and provides opportunity to contribute their knowledge and experience to others who need them. WIC: Women, Infants and Children (WIC) Supplemental Feeding and Nutrition Program provides food vouchers, allowing them to purchase milk, eggs, cheese, juice and iron fortified cereals and formula. The WIC coordinator provides nutrition education and counseling to all program participants on a monthly basis. Senior Dining Program: provides weekday hot lunches to seniors and disabled adults. VITA: volunteers provide free basic tax preparation for low- to moderate-income households.

Westbay Community Action is a charitable organization serving the people of Kent County. They help with food, utilities, health, safety, and education. West Bay CAP provides a variety of resources to individuals, families, and seniors in need. Emergency and Case Management services provide intakes, Senior Services Case Management by their Elder Case Management Team), and supportive housing (subsidized rental units with case management services). Food services include holiday programs, intake, congregate meals for seniors and disabled adults, Westbay Marketplace food pantry and WIC (Women, Infants and Children monthly food package and nutrition counseling to pregnant women, new moms and infants and children up to age 5 years old). Home and Utilities services provided include: Good Neighbor Energy Fund, Lead Center, LIHEAP heating assistance, Low Income Household Water Assistance, supportive housing subsidized rental units with case management, and weatherization home energy programs. Senior services include: the RSVP (Retired and Senior Volunteer) Program which matches senior volunteers with nonprofits; congregate senior dining program serving weekday hot lunches to seniors and disabled adults; Senior Information and Safety Programs (health and safety) and case management services. Westbay also offers VITA (Volunteer Income Tax Assistance) which provides free basic tax preparation for low- to moderate-income households.
