Description
Evergreen Assisted Living is a 25 bed, co-ed Assisted Living Residence for adults and seniors with behavioral healthcare needs who require assistance with everyday activities. Staff assists residents with coordination of services and encourage participation in various activities in the local area. Evergreen is staffed with dual-licensed Nursing Assistants and Medication Aides. A Registered Nurse meets monthly with residents to evaluate healthcare needed and coordinate service planning. Easily accessible to public transportation.
