Heritage Hills joined the Health Concepts family of caregivers in 2012. Our skilled nursing and rehabilitation center houses up to 95 patients each day who come to us for short-term rehabilitation, post-acute care, or long-term care for those with a chronic illness, dementia or a need for assistance with multiple activities of daily living.
