They accept all third party insurances including Medicaid, Medicare and RIte Care. Self-pay and sliding scale fee are available for those who do not have health insurance and meet financial eligibility requirements

One of four community-based health centers offered by CCAP. Provides a full range of primary health and behavioral health care services including OB-GYN, Pediatrics, Geriatrics, maternal child home visiting, HIV Testing, Case Management, Mental Health Counseling/Substance Treatment including MAT and Oral healthcare, as well as eligibility assessments for state-funded programs.

They have a staff of caring, experienced Licensed Independent Social Workers and Licensed Chemical Dependency Counselors provide individual, couples, family and group therapy.

From time to time everyone needs a helping hand. CCAP offers a full range of services to help individuals and families keep a warm roof over their heads and food on the table.
CCAP offers a full range of behavioral health counseling services to help individuals and families develop healthy ways of coping with adversity and resolve problems that have created obstacles to leading full lives. Their staff of caring, experienced Licensed Independent Social Workers and Licensed Chemical Dependency Counselors provide individual, couples, family and group therapy.
The CCAP Food Bank provides nutritious food and support services to people in need with compassion, dignity and respect. We are a Choice Food Bank. Their Choice Food Bank enables people to “shop” through a food selection process, much like they would in a grocery store, rather than receiving pre-packaged bags. The Choice Bank creates a dignified experience that encourages the selection of healthy foods.
No appointments are necessary during regular hours – they serve on a “first come first served basis” If you are a new client, they ask that you identify yourselves so they can start your client paperwork. New clients to CCAP’s Food Bank start with an intake interview that determines household size and dietary needs along with other CCAP services they may need such as health care or heating assistance.
