Own your Health RI- Evidence-based health and wellness workshops, offered through FHH's Own Your Health Coalition, help older adults live fuller and healthier lives. FHH offers free workshops including: Chronic Disease Self Management, Chronic Pain Self Management, a Matter of Balane (fall prevention) and Tai Chi Quan. These programs are designed for those seeking to better maintain and stay informed about their health as well as be their own best advocates with their medical professionals and caregivers. The workshops are given through a peer to peer model, so participants learn from those managing the same health concerns., Central Providence Healthy Aging Network (CP-HAN)-the Central Providence Healthy Aging Network is a "village" membership club that connects older adults with community resources. Our goal is to help older adults maintain their independent and quality of life. The CP-HAN connects older adults with an array of opportunities and supports, including daily meals, exercise, enrichment, transportation, minor home repair, home visiting, nutrition education, access to food support, healthcare advocacy. CP-HAN utilizes staff, volunteers, and community partnerships to keep seniors active and healthy. CP-HAN is a partnership with ONE Neighborhood Builders and part of the Central Providence Health Equity Zone initiative. Foster Grandparent Program (FGP)-
