Description
Responsible for maintaining the State's vital records system. Collects, analyzes and reports data pertaining to births, deaths, marriages, divorces and various health related statistics. Issues certified copies of vital records. Birth, death and marriage certificates are also available elsewhere: - NEW: For people born in Rhode Island January 1, 1980, or later, most cities and towns can now issue an electronic certified copy of a birth record regardless of where the birth happened within the state. - For birth records prior to January 1, 1980, and death or marriage records for all years, (except as noted below in option 5), go to the city or town hall where the birth, death or marriage happened or the city/town of residence at the time of when the event happened. Click here for a list of city and town halls. - Send a completed request application to the State Office of Vital Records, 3 Capitol Hill, Room 101, Providence, RI 02908. The request must be accompanied by a readable copy of a valid government issued picture identification such as a driver�s license, passport, etc. - Come in person to the State Office of Vital Records, Department of Health, Monday through Friday, 12:30 p.m. � 4 p.m. You must have a valid government picture identification with you. - Birth and marriage records 100 years or older and death records 50 years or older are considered public records and may be obtained from the city or town hall where the event happened or through the R.I. State Archives
