Cedar Crest Subacute and Rehabilitation programs, a 156 bed medical center, were designed for patients with physical, medical and cognitive impairments who would benefit from patient-centered care and an interdisciplinary team approach. Our goal is to work with patients to achieve their optimum level of functioning. When those treatment goals are met, the patient is ready to move on. Treatment plans can include physical therapy, occupational therapy and speech-language therapy, along with nursing care oriented to the needs of rehabilitation patients. The patients most appropriate for this program are those whose diagnoses are orthopedic conditions, stroke, neurological disorders, or those who have experienced general weakness or de-conditioning following surgery or an illness. A private, nonsectarian, not-for-profit behavioral healthcare organization that supports individuals in their recovery from mental illness and co-occurring disorders (i.e., substance use, developmental disability, related chronic health conditions). Provides access to support and resources for individuals to lead self-determined, independent, and fulfilling lives as contributing members of the community. Assists people of all ages to improve their behavioral health, well-being, and quality of life. Licensed professionals conduct individual assessments and develop person-centered treatment plans. Individuals are seen by a psychiatrist or a psychiatric nurse practitioner for medication management, and individual and group therapy are available. Providence Center-(Prairie) Provides: behavioral health services ranging from emergency services, general and intensive community-based outpatient treatment, substance use treatment, and other services. They may offer vocational and supported employment services, navigators who can help you get health insurance, tax preparation assistance, and housing supports. If receiving services as a child, assistance with transition to adult services or programs are provided within or outside the agency. The Community Mental Health Centers are regionally based as indicated below. However, as a Rhode Island resident, you may access services at any CMHC. Anyone experiencing a crisis can walk into a Community Mental Health Center during their normal business hours and they will provide treatment. For after-hours emergency treatment, call the Center’s emergency number or call 911. Flex buses are available by appointment. Call RIPTA 24hours before to reserve at 1-877-906-3539. LogistiCare (LGTC) manages non-emergency medical transportation (NEMT) for medicaid members. You can receive a ride to a medicaid-enrolled medical practitioner/provider. The Veterans Transitional Supportive Program (VTSP) is a collaborative effort by the Rhode Island Veterans Home and the U.S. Department of Veterans Affairs to assist homeless veterans in their return to community living. The objectives of treatment are to have each veteran achieve an optimal level of functioning mentally, physically, vocationally and socially so when community housing is located they will not experience homelessness again. Residents typically can remain in the program from six months to a year where they are assisted with substance abuse treatment, work therapy, case management services, individual counseling, medical and psychiatric treatment and discharge planning. Each veteran is expected to develop an individualized treatment plan and work with his/her case manager to resolve those issues which contributed to his homelessness.

Under federal regulations, ADA Paratransit Service must operate within a 3/4-mile corridor on either side of a fixed route. ADA service operates during the same hours that the fixed route bus runs. All ADA trips must start and end in the corridor.

People who have been laid off, who aren’t sure how to navigate unemployment, who may not know how to bridge the gap until they start receiving unemployment checks, are all encouraged to call during their extended hours or video conference via Zoom. Financial Coaching Crash Course: We are offering a Crash Course version of our coaching program for those impacted by COVID-19.
