It can also help identify children who may need further assessment, intervention and/ or services at an early age to promote positive outcomes in kindergarten and beyond.

Case/Care Management at Westbay Community Action includes: FCCP: A voluntary program for children who are at risk for abuse or neglect or who have serious emotional disturbance. FCCP connects families with natural supports in their extended families and in the community. FCCP does not provide direct services. Instead the program provides referrals, support, guidance, and advocacy. Elder Case Management: Provides assessment, monitoring and protective services for seniors. Warwick Case Management: Offers long-term support services to at-risk Warwick residents.

Respite provides temporary care for children with special needs. This allows families time away from caregiving responsibilities. Families have the ability to identify the caregiver and when services are needed. Respite can range from a few hours to overnight care.

Part of Community Care Alliance, the Agape Center is a community based HIV/AIDS support and resource center whose mission is to provide services, resources, and advocacy for persons living with or affected by HIV/AIDS and to promote community awareness and prevention of the disease. Their Agape Drop-In Center provides resources in a safe, confidential setting where people living with HIV/AIDS are treated with dignity and respect. Westbay’s Lead Center provides a Case Manager who will walk families through the application process. This program provides forgivable loans to mitigate the hazards of lead-based paint and to make your home a healthier and safer place to live. The LeadSafe Homes Program is available to homeowners and landlords at no cost to qualifying customers. The Lead Center provides community outreach and education about lead-related issues. Westbay’s case managers can make home visits to assess for lead hazards and/or provide education to families with children under six years of age with no referral. Assessment The Lead Center offers free in-home lead assessments to any property owner, landlord, or tenant worried about lead paint hazards. Assessments are especially important for households with young children and for those who are planning to renovate. Case Management The Lead Center provides non-medical case management for families of children with lead poisoning. Case managers help families coordinate medical care, developmental screenings, and nutritional counseling. They can also offer assistance with housing inspections or relocation.

Acute Stabilization Unit The ASU, as we call it, provides short term, residential services for the person who is having a mental health and/or substance abuse emergency, but who does not need hospitalization. Referrals are made through our Emergency Services Team, hospitals and other sources. Intensive Outpatient Program Successful independence is the goal of our recovery-focused Intensive Outpatient Program for those with co-occurring disorders. You’ll get care and guidance from our skilled counselors in a safe, supportive setting. Opioid Treatment Center of Excellence We believe that treatment works. Meet with a Peer Specialist who will walk you through the steps to getting help with opioid addiction. Outpatient Counseling Services Counseling is provided in an office-setting for addictions, including problem gambling. DUI Court Ordered Treatment is also available through our Outpatient Counseling program. Partial Hospitalization Program (PHP) Our new Rise to Recovery PHP program helps individuals and family members recover from the negative consequences and life issues associated with substance use and addiction. Residential Substance Use Treatment Treatment and around-the-clock support provides men with the opportunity to focus on their recovery from chemical dependency. Our supportive services can become a part of your safety net after discharge as well. Recovery Housing Sober housing and support services are available who have recently completed a residential treatment program to help them along the road to recovery. They provide clinical and support services for individuals and families with long-term needs associated with mental illness and substance abuse. Their goal is to help people live as independently and as fully integrated within the community as possible These might include basic needs assistance to address emergencies— counseling, treatment, support services and residential programs for individuals and families impacted by serious mental illness and addiction— emergency and supportive housing to prevent homelessness and promote stabilization— alternative education and services for families with young children and adolescents— and employment, financial literacy and vocational training to improve self-sufficiency.
