Westbay’s Supportive Housing Program operates 6 subsidized rental units in Warwick and West Warwick. A case manager links tenants to services at Westbay and at other agencies. The case manager meets with them regularly to help with basic skills and support. The goal of the program is to help tenants achieve independence.

Full Service Dental for Adults Living with HIV and individuals who identify as trans in the community. Providence-based dental office has been established to meet the dental needs of HIV positive adults in our community. Funding from the Ryan White (part C) grant enables Thundermist to provide diagnostic, therapeutic, and preventative dental services to HIV positive adults, who might not otherwise have access to care. Housing programs that make rental housing more readily available to homeless people and/or low-income individuals and families (those below 50% of the area's median income). Some complexes or housing units may be reserved for low-income older adults, people with disabilities and/or other special populations.

Programs that pick up and impound or return to their owners domestic animals who have strayed from their homes. Also included are programs that enforce leash laws and animal sanitation ordinances and those that handle complaints regarding domestic animals who have bitten someone or are otherwise making nuisances of themselves. Most animal control programs are unable to handle nuisance wildlife unless the animal is sick or shows symptoms of rabies.

Eleanor Slater Hospital is a two-campus hospital. The hospital provides long -term acute and post -acute hospital level of care to patients with complex medical and psychiatric needs. ESH emphasizes collaboration among its leadership, physicians, nurses, and rehabilitative staff reviewing all processes associated with operations and quality care. Processes are continually modified or redesigned resulting in higher quality care for our patient population and improved operations.

Emergency Shelter. This new program exclusively serves unhoused individuals over 55, and is located in the same facility as Amos House's Emergency Family Shelter for individuals ages 55 and up This program provides emergency and stabilizing wrap-around services to 40 individuals. These services include: daily meals, case management, housing location, medical coordination, behavioral health support and more. Street Outreach Program: Outreach Team works with unsheltered individuals who are at risk for violence, providing them with vital supplies, harm reduction materials, case management, and access to housing and recovery services. Housing Assistance: including help covering utility bills, rental assistance, and other costs associated with securing housing. Funds are limited for this program. Hygiene Packets: Each year, Amos House distributes more than 15,000 individual hygiene packets to men, women, and families. IDs and Birth Certificates: Case Managers help those in need cover the fees and navigate the process to obtain an ID or Birth Certificate. A Hand Up: A day labor program for individuals who are panhandling or at risk of panhandling, AHU offers cash payment for daily work and connects them to supportive services intended to help them access other options and greater opportunities.

Services include: Breast feeding instruction and support; Wound care; Post partum care and monitoring for complications; Nutritional support; Assessment for maternal depression. Home care is provided to infants and children who have had surgery, an injury, or an illness. Services Included: Wound care; Cast care; Asthma care; Care for new diabetics; Care for seizure disorders; Medication administration; Physical, occupational, or speech therapies; Instruction in use of home medical equipment; Care for premature infants from Special Care Nurseries and Neonatal Intensive Care Units.

CrossRoads RI's Harold Lewis House is designed to provide elder individuals with a safe, secure, and affordable place to live. Harold Lewis House offers 14 beds of permanent supportive housing for disabled adults aged 50 and over who are homeless.

Because issues associated with serious disabilities can be physical as well as emotional, SNERC sponsors support groups for patients who have had a brain injury, stroke or spinal cord injury as well as host a family education and orientation group. Stroke Support Group The Fatima All S.T.A.R.S. (Stroke, Together, Advocacy, Recovery, Support) invite you to join "HOPE after STROKE" meetings which take place in the rear of the 4 South dining room at Fatima Hospital. Spinal Cord Support Group Meetings take place in the outpatient therapy gym at Southern New England Rehabilitation Center’s Providence location at St. Joseph Health Center, 21 Peace Street. The gym is located on the third floor on the west wing.

Outpatient programs are for those seeking mental rehab or drug rehab, but who also stay at home every night. The main difference between outpatient treatment (OP) and intensive outpatient treatment (IOP) lies in the amount of hours the patient spends at the facility. Most of the time an outpatient program is designed for someone who has completed an inpatient stay and is looking to continue their growth in recovery. Outpatient is not meant to be the starting point, it is commonly referred to as aftercare.

A non profit agency dedicated to the education of Rhode Island youth regarding the dangers of drinking and driving. Provides safe transport to/ from school sporting events, playing a short educational video about the dangers of underage drinking/ driving. Assists schools by creating mock DUI scenes for educational purposes.

Part of Community Care Alliance, the Agape Center is a community based HIV/AIDS support and resource center whose mission is to provide services, resources, and advocacy for persons living with or affected by HIV/AIDS and to promote community awareness and prevention of the disease. Their Agape Drop-In Center provides resources in a safe, confidential setting where people living with HIV/AIDS are treated with dignity and respect. Services: HIV and Hepatitis C testing (by appointment) Lunches are served Monday and Wednesday, 12-1 PM Weekly Food Pantry Monthly Personal Care, Cleaning and Household Items Case Management services, in English and Spanish Support and resources Eligibility: Clients must be HIV+ and live in Rhode Island.
