CharterCARE visiting nurse service is unique in that the clinical staff practices primary nursing. They will systematically assess, plan, deliver, and continually evaluate an individualized plan of care for each patient.
Skilled Care: Clinical staff provides skilled training in the use, care and management of various medical devices including; G-tubes, J-tubes and foley catheters. We provide training in and assistance with injection therapy, oxygen therapy, ostomy care, tracheostomy care, the administration of insulin for diabetics, medication management, disease management and wound care.
Advance Wound Care
Every one of our nurses has been trained and is proficient in performing negative pressure wound therapy, a state of the art technology proven to significantly reduce healing time for chronic wounds. (ie: Wound Vac, Renosys and Engenex).
Telemonitoring
Our telemonitoring program is an adjunct to nursing visits and physician care. Upon the physician order, equipment is installed in the home for the patient. The equipment allows the patient to take daily measurements of weight, blood pressure, pulse and oxygen saturation. Results are monitored daily by an RN.
Nutritional Services
CharterCARE Home Health utilizes registered dieticians (RDs), food and nutrition experts who have met American Dietetic Association (ADA) criteria to earn the RD credential. RDs are available to assess a patient's nutritional needs, develop and implement nutrition programs, and evaluate and report the results.

Support and advocacy for the blind. Monthly meetings focus mainly on advocacy activities such as legislative and public policy development and public education. However, peer support for individuals is available and provided as needed. Purpose of the group is to foster positive attitudes towards blindness for both the blind and sighted communities.

The Greater Providence Chapter of the NFB of RI meets on the second Saturday of each month, except for the months of July and October from 10:30 a.m. to 1:00 p.m. and they are preceded by a social half hour at 10 Office Parkway, in East Providence. The Mount Hope Chapter of the NFB of RI meets on the third Thursday of each month, except for October and December from 5:30 PM to 7:30 PM, preceded by a social half hour. Meetings are held at the Rogers Free Library, located at 525 Hope Street, in Bristol. Those using the Ride program should ask to be dropped off and picked up at the Thames Street entrance, located in the back of the building. Caring for a loved one can often be overwhelming, so the CareBreaks program offers the gift of time. It can help family caregivers arrange for, and perhaps help pay for, a break. A break for a caregiver may include bringing a volunteer or paid worker into the home, arranging for your loved one to attend a safe day program, or even placing your loved one in an overnight facility for a short period of time. CareBreaks may be used occasionally or at regularly scheduled times. This gives the caregiver time to shop, get medical care, attend to personal matters, rest, vacation, etc.

Centers for Independent Living teach a wide range of skills needed to live independently and manage life with a disability. Skills training is available to persons with disabilities in almost any topic where there is a need for more independence. CILs work with individuals to increase their skills which can help them gain more control of their lives.
Skills training can include basic everyday responsibilities such as: self-care, meal preparation, budgeting, and arranging transportation. It also can address housing needs, teaching individuals the importance of self-advocacy, one’s need for accommodations and/or rights in the community.
***Life skills training is goal directed and does not involve long-term case management or therapeutic intervention.
