Concentra provides physical therapy services to patients, regardless of how the injury occurred. Many patients come to Concentra after a work-related injury, and works directly with the physician to develop a treatment plan to return them to full-function and back to work in a timely manner.

Services: A small independent day school founded in 1999 to provide a unique educational environment for children with multiple learning differences. The Ocean Tides School Providence Campus is an alternative day school for students in grades seven through twelve. A collaborative group of local comprehensive and specialized information and referral providers who have agreed to coordinate information and referral service delivery to provide seamless access to information about community resources via the three digit number assigned for this purpose, 211. Depending on the size and nature of the community, 211 systems may consist of only a few organizations that provide information and referral or may include a diverse array of organizations that furnish some type of I&R. Some of these organizations may supply a very specialized type of I&R service (e.g., for people with disabilities or older adults), while others may provide a comprehensive range of assistance. The coordination of all I&R services and programs within a community and, where relevant, beyond, constitutes the 211 system.

Home Care We all need a little help sometimes. Home care services help people age strong in the community, connecting them with in-home supports, adult day health programming, and/or assisted living services. For older adults with care needs, the @Home Cost-Share program is available to help with some care expenses. Eligible participants can access in-home services – such as help with housekeeping, meal preparation or personal care – and/or community-based programming at adult day health centers at a reduced rate. You may be eligible for the program if you are age 65 or older, meet income guidelines, and need some assistance in the home with your personal or health care.
