Long Term Services and Supports (LTSS) serves people who have disabilities or chronic care needs in the setting ideal for them and their families. Services may be episodic or on-going. Services may be provided in a person's home, the community (for example, shared living or assisted living), or institutional settings (for example, intermediate care facilities, hospitals, or nursing homes) to optimize their health and retain their independence. People of all ages may need long term care. Foundations Academy (formerly the Regional Alternative Program): Provides special needs students - diagnosed with behavioral and emotional disorders - with an alternative to traditional schools. Foundations Academy (formerly the Specialized Alternative Program): Students who learn differently and have different abilities need the help and guidance of educators who are specially trained to develop and maximize those abilities. Educators in the Foundations Academy work to identify a student’s skills and their potential for learning new skills. Educators, parents and students then develop a progressive plan tailored to an individual’s educational needs and social well-being.

Program provides a noontime meal five times per week to qualified individuals delivered by volunteers.

A federally funded program that works to improve the health of elderly people age 60 and older who meet income eligibility requirements by supplementing their diets with nutritious USDA commodity foods. The program provides food and administrative funds to states, typically departments of health, social services, education or agriculture. The state agencies store CSFP food and distribute it to local public and private, nonprofit organizations that determine the eligibility of applicants, distribute the food, and provide nutrition education.

Grief and bereavement support groups are held at various times and locations throughout the year to support individuals and families who have lost a loved one. Coming together with others to share similar feelings and thoughts is beneficial to the grieving process. The support groups are a 6-session series led by the hospice chaplain, who teaches healing and coping strategies. Other programs offered for families and friends who have lost a loved one include: Circle of Love and Light held in the first week of December. Annual Interfaith Service, held each spring.

Women & Infants' Breast Health Center is a specialized resource for patients and a consultative service to physicians offering comfortable facilities, advanced technology, specialized expertise, prompt evaluation, and a full spectrum of contemporary treatment options. The Breast Health Center has a perfected model program for total breast care, developed all of the necessary educational and support systems and assembled a multidisciplinary team of women's health specialists. The Institute for the Study and Practice of Nonviolence’s (ISPN) mission is to teach, by word and example, the principles and practices of nonviolence, and to foster a community that addresses potentially violent situations with nonviolent solutions. To work to build Dr. King’s ideal of the nonviolent Beloved Community. ISPN’s 2017-2019 AmeriCorps program focuses 20 Americorps members in school based nonviolence programs which work with students to develop attitudes, skills, and behaviors to further a movement to achieve lasting reductions in violence that occurs in our communities. ISPN AmeriCorps members are implementing nonviolence education curriculum for at least 1,000 economically disadvantaged students in ten schools over the course of the year. Teams of two AmeriCorps members provide nonviolence education for five classrooms of 20 students, for a total of 100 students trained in each participating school. The members teach one or two classes of students each day for four days a week. When not teaching, members work to mediate conflicts between students who are having problems with each other. They help students who have been assigned to in-school suspension to develop strategies to navigate problems at school in more effective ways.

Programs within the formal criminal justice system that provide facilities for the detention of people who have been charged with a crime and are awaiting trial, and/or the confinement, treatment, employment, training and discipline of people who have been sentenced to imprisonment after conviction for a criminal offense.
