Personalized Case Management (Non-Medical): Case managers work with clients to understand and get access to services they are eligible for and to help them take control of certain aspects of their lives. Case managers provide direction on things such as assistance with paperwork and connections to medical, dental, and mental health providers. Case managers also provide information regarding housing, legal referrals, food resources, wellness programs, emotional support, education, medication, clinical trials, and statewide service organizations. Oral Health Care Services This program provides access to dental insurance for individuals who do not currently have it. In partnership with Delta Dental, APRI clients are enrolled in an insurance plan that covers teeth cleanings and allows for a maximum annual coverage for pre-approved dental services of $1,750. Medically necessary exceptions that exceed this amount are considered on a case-by-case basis. All payments are made directly to the provider on behalf of the client. Emergency Financial Assistance Program Clients may be eligible to receive financial assistance for a variety of needs, including medical costs, rent, heat, and other utility expenses. Verification of income and expenses, along with completion of an Emergency Financial Assistance application is necessary. Clients must apply for funding, with an annual limit of $2,500. All payments are made by APRI directly to the landlord or service provider, as appropriate. Bus Passes & RIDE Tickets Limited supplies of RIPTA bus passes and RIDE tickets are available for persons in need of transportation to medical visits. Clients who are able to obtain their own transportation are not eligible. Clients who are disabled are encouraged to apply for bus passes through RIPTA directly, or we can assist you in applying. Mental Health & Substance Use Counseling Services (Outpatient) Clients can receive counseling by providers who specialize in HIV, relationships, substance use/abuse, and other issues. Services are provided to individuals regardless of insurance status. (Counseling services are also available for HIV-negative individuals. Please contact us to discuss and set up an appointment.) Afia Center for Health & Wellness APRI is proud to offer of a variety of services through our Afia Center for Health & Wellness, co-located at Open Doors at 485 Plainfield St. in the Silver Lake neighborhood of Providence. Afia is a drop-in center that operates on Wednesdays and Thursdays 12-3 p.m. and offers hot meals; a food pantry that also includes access to personal care items; outings and group activities; and much more. New members must meet with APRI’s program coordinator to determine eligibility for services. There is no fee associated with the services offered through the Afia Center. Nutritional Support Clients who have a statement or evaluation from their medical provider indicating the need of a nutritional supplement are provided cases of Ensure Plus or Glucerna. Clients who have health insurance are eligible for this program if they can show written proof that they cannot get nutritional supplements from their own pharmacy. APRI’s licensed dietitian provides counseling for a client who is prescribed nutritional supplements. ADAP Enrollment & Medication Management We can assist clients in filling out and submitting paperwork for the state’s AIDS Drug Assistance Program, which guarantees access to HIV and other necessary medications. In addition, APRI offers services from a registered provider who can discuss medications with clients as needed. Legal Services & Referrals For older individuals living with HIV, end-of-life planning can be critically important. APRI partners with local attorneys who specialize in elder and estate planning to offer the creation and notarization of documents such as: Last Will and Testament, Durable Power of Attorney for Financial Matters, RI Health Care Power of Attorney, RI Living Will, and HIPAA authorization form. Clients need only be Ryan White eligible to receive these services. Housing Advocacy Stable housing is a crucial part of maintaining one’s overall health. We work with clients who are homeless, housing unstable, or who are seeking a change to their housing situation to apply for assistance, search for different housing options, and more. Health Insurance Premium Assistance APRI can assist clients who need help paying for their own health insurance premiums or COBRA payments for up to six months. If longer term assistance is needed, we will help a client transition to another insurance provider through Health Source RI.

Services: Provides intake/outreach in areas of housing (including fuel assistance, home improvements and placement), emergency services (including domestic violence, fire and vandalism), shut-in, home-bound. Emergency food is available. Community relations involve workshops, urban gardening. Youth program operates during after-school hours, weekends and school vacations (offers counseling on college preparation; placement and career awareness; co-ed basketball for ages 13-18 years). Elizabeth Buffum Chace Center offers short-term, safe, confidential shelter for victims of domestic violence and sexual assault and their children. Residents are provided with all basic needs and are encouraged to participate in a program that includes education about domestic violence, parenting and financial literacy. Provides specialized children’s services, advocacy and referrals. 16 bed capacity, NOT handicap accessible. Services Available: Case management; Crisis intervention (on-site); Domestic violence support and education; Housing search and education; Budgeting and financial literacy; 3 meals per day. Important Policies: Rent is NOT required; Medications are locked and stored in a secure location and available to guest as prescribed; Smoking allowed in designated outdoor areas; Drug/alcohol free environment; Children must be supervised at all times; Agency follows least-harm model. The Elizabeth Buffum Chace Center offers a transitional housing program for families who have been affected by domestic violence and sexual assault. Our program provides safety, stability and support, while clients take the next steps toward self-sufficiency. The Elizabeth Buffum Chace Center is excited to announce it's new Permanent Supportive Housing program. Housing with no end date with wrap-around services available if needed.

They work to match businesses that have current, unmet hiring needs with qualified, unemployed and underemployed job seekers. They provide training and workforce development, working with over 80 different employers.

Services: Provides educational and training information for post secondary education, financial aid and admissions information, application assistance in plus career counseling and assessment, referral to other social services and advocacy. Hospital charity care requirements cover those essential services necessary to diagnose, correct, or cure a medical condition that endangers life, causes pain and suffering, results in illness or infirmity, or causes a physical deformity or malfunction. Both inpatient and outpatient services are included. To receive full charity care, patients annual family income must be up to and including 200% of the Federal Poverty Level. For partial charity care, patients annual family income must be between 200% and 300% of the Federal Poverty Level may receive assistance based on a sliding scale. In addition to income, hospitals have the option to consider other assets. Also must be denied from Medicaid first depending on income. Heatherwood Rehabilitation & Health Care Center offers short-term rehabilitation, long-term, respite, hospice and dementia care.

Currently the only needle exchange program in the state. Functions as a Harm Reduction program including needle exchange, drug treatment referral, free condoms, free clothing and free HIV testing on-site. **Van also brings needle exchange to other areas of Rhode Island** Woonsocket: Tuesdays from 2:30 - 4:30 PM (Arnold St., next to the Soup Kitchen.) Newport: Fridays from 2:30 - 4:30 PM. (Near MLK Center and Wheatland Blvd.) E.N.C.O.R.E. E - Education on HIV prevention. N - Needle exchange to reduce the risk of transmitting HIV and other comminicable diseases. C - Counseling on reducing risk, HIV testing, following through on medical care, and substance abuse treatment. O - Outreach into the community to identify potential clients for the E.N.C.O.RE program. RE - Referral to agencies and programs by request from the client.

Provides free, voluntary, confidential home visits to pregnant women and families with young children, birth to age three. Trained nurses, social workers, and community health workers meet with families in their homes to talk about their needs, answer questions they may have, and conduct comprehensive assessments. Home visits are based on the needs of the family. Visit topics may include health education and connections with appropriate healthcare services, human services, and community resources. If a child does not have a medical home, we can help the family find one.

A Medicare Advantage is another way to get your Medicare Part A and Part B coverage. Medicare Advantage Plans, sometimes called "Part C" or "MA Plans," are offered by Medicare-approved private companies that must follow rules set by Medicare. If you join a Medicare Advantage Plan, you'll still have Medicare but you'll get most of your Part A and Part B coverage from your Medicare Advantage Plan, not Original Medicare. These "bundled" plans include Medicare Part A (Hospital Insurance) and Medicare Part B (Medical Insurance), and usually Medicare drug coverage (Part D). Most of them usually offer extra benefits. With a Medicare Advantage Plan, you may have coverage for things Original Medicare doesn't cover, like fitness programs (gym memberships or discounts) and some vision, hearing, and dental services (like routine check ups or cleanings). Plans can also choose to cover even more benefits. For example, some plans may offer coverage for services like transportation to doctor visits, over-the-counter drugs, and services that promote your health and wellness.
