The Fogarty Center offers individual Person-Centered services for job seekers with intellectual/developmental disabilities by offering a comprehensive progression of employment-related services. These services include; Discovery, Career Exploration, Job Trial/Assessment, Placement, Job Coaching and Monitoring. The Fogarty Center also supports adults by planning meaningful activities throughout the day, based on personal preference, within a variety of settings including day programs as well as locations within the community. Founded in 1971, CODAC Behavioral Healthcare is Rhode Island’s oldest and largest non-profit, outpatient provider of treatment for Opioid Use Disorder (OUD). With seven community-based locations, as well as programming at the Rhode Island Department of Corrections (RI DOC), CODAC is well-positioned to deliver services wherever they are needed across the state. CODAC offers psychiatric and mental health services at all of their locations. After an initial intake screening you may be referred to an on-site psychiatric practitioner for further evaluation and treatment. Individualized treatment and recovery plans might include medication management supervised by the psychiatric practitioner and ongoing co-occurring counseling with clinical staff. It is not unusual to have both mental health disorders and a substance use disorder. Because the two conditions are so closely linked, CODAC has responded to offer you effective treatment and recovery programs to address both disorders concurrently.

The Domestic Violence Resource Center of South County works toward a future free from violence. They work collaboratively to offer safety, support, advocacy, education and a network of services to residents of Washington County. DVRCSC has a confidential safe home for women who face homelessness as a result of domestic violence. Additional services include a Drop-In Advocacy Center, Support Groups and One-on-One Counseling by appointment. They offer education and outreach on the subject of healthy relationships, dating violence, cyber stalking and techno-abuse. DVRCSC can provide Court Advocates at McGrath Judicial Complex in Wakefield.

A comprehensive, State licensed adult day care program to provide medical, social and rehabilitative services to chronically impaired elders and disabled adults. Services include: nursing supervision, personal care, counseling, caregivers support/respite, exercises, activities, specialized therapies and nutrition. The Adult Day Services Program provides an array of supportive services designed to upgrade and increase the activities of daily living for older persons dependent upon continual family support and supervision. The major goal of the program is to reduce the potential for institutionalization of dependent elderly individuals otherwise at risk, and to increase their quality of life through the enhancement of their activities of daily living. The program provides supervised supportive care for frail elderly persons to meet the needs of the functionally impaired individual. The program also provides respite and emotional support to family caregivers. Income Source: Towns of South Kingstown, Narragansett, R I Department of Elderly Affairs, R.I. Department of Human Services, client fees and private contributions.

During this crucial stage of a child’s growth and development, the Seven Hills EI professionals work with families as a team to help them help their children where they learn best: at home and in community settings. Seven Hills EI program partners each family with a Service Coordinator who works with a team to assess the child’s development based on medical history, observation of the child performing a daily routine, and learning about the child’s likes, dislikes, successes, and difficulties. If the child is eligible for EI, the team will work with the family to develop an Individualized Family Service Plan (IFSP), to develop priorities, goals and outcomes. The Seven Hills professionals make it as convenient as possible for families to schedule their EI visits in the child’s natural environment—at home, in a child care setting, or during a community activity. The family’s multidisciplinary EI team may include educators, therapists, social workers, and nutritionists. Parent Consultants (PCs)—parents whose children have been through the EI program—are also available to share resources, offer support, and facilitate networking.

Outreach Services are offered to the senior (60+) and disabled adult population of Scituate to assist with applications for financial and benefit assistance, including SNAP (Food Stamps), Fuel Assistance, Housing, and Medical Insurance, as well as information or referral to Meals On Wheels and the Food Pantry. Transportation Coordinator at 781-545-8872. Rides within Scituatte should be scheduled at least 2 days in advance; out-of-town medical rides should be scheduled at least 5 days in advance. For new clients, a brochure should be requested and signed in order to begin to schedule rides with the Senior Center. Meals on Wheels and the Food Pantry. The Scituate Senior Center/Council on Aging provides transportation services to Scituate adults 60+, and those who meet ADA requirements for general rides within Scituate as able, as well as rides to and from Medical appointments in the area, including out-of-town as far as Boston. Scheduled trips to the local markets. Scituate Food Pantry: Please call if you need transportation assistance.
