Programs that provide a full range of supportive services for terminally ill individuals who are in the final stages of their illnesses and for their families. Services may include medical care, pain and symptom management, home nurse visitation, case management, emotional and spiritual support, and bereavement services for the patient and members of the family. Hospice care may be provided at home, in a freestanding hospice facility, a hospice unit of a hospital or in long-term care facilities such as nursing homes. In order to qualify for Medicaid or Medicare reimbursement, an individual must have a life threatening illness and must be certified by their physician to have entered the last six months of life.

Hospital charity care requirements cover those essential services necessary to diagnose, correct, or cure a medical condition that endangers life, causes pain and suffering, results in illness or infirmity, or causes a physical deformity or malfunction. Both inpatient and outpatient services are included. To receive full charity care, patients annual family income must be up to and including 200% of the Federal Poverty Level. For partial charity care, patients annual family income must be between 200% and 300% of the Federal Poverty Level may receive assistance based on a sliding scale. In addition to income, hospitals have the option to consider other assets. Also must be denied from Medicaid first depending on income. The Volunteers in Schools program is an integral part of the Portsmouth School Department. Volunteers are recruited and trained to make major, system-wide contributions in the areas of tutoring, health screening, kindergarten registration, classroom assistance, enrichment, student publishing and print media reproduction.
