13 room home for community based program is located on the periphery of Pawtucket's business district. The primary objective is to provide residential services for adolescent males. As members of the community, the residents are afforded the opportunity to participate in a variety of educational, recreational and cultural programs. All residents attend school locally with staff providing supplementary and remedial academic assistance. The program provides a clinican licensed at the independent level which may include a licensed psychologist, a licensed mental health professional, a licensed independent social worker or a licensed marriage and family therapist. The clinician will have weekly individual and group contact with each resident and provide consultation to staff, DCYF caseworkers and the resident's parent or guardian. Family therapy will also be provided when appropriate. The clinician will also provide consultation with other mental health professionals, school personnel and assist with crisis intervention while making appropriate treatment referrals.

Provides comprehensive rehabilitation services to persons, 18 - 100, who are blind or severely visually impaired, through a center-based program, regional satellites, or community services. Included are daily living skills training, orientation and mobility, counseling, diabetic program, summer camp, low vision services, workshop training and employment, a closed circuit radio information service, volunteer services, speakers bureau, tapes, Braille material, aids and appliances. The substance abuse task force identifies and addressed the issue of substance abuse in the city and its mission is to develop, implement, and advocate for effective community-based prevention initiatives.

The Supplemental Security Income (SSI) program provides monthly payments to adults and children with a disability or blindness and who have income and resources below specified amounts. SSI payments can also be made to people 65 and older without disabilities who meet the financial limits. People who have worked long enough may also qualify to receive Social Security disability or retirement benefits as well as SSI.

The FBI's Internet Crime Complaint Center, or IC3, is the reporting mechanism to submit complaints regarding cyber crimes to the FBI. Information is analyzed and disseminated for investigative and intelligence purposes to law enforcement and for public awareness. The IC3 accepts online Internet crime complaints from either the actual victim or from a third party to the complainant.

Shelter Policies: All calls for service must be filed through the Warren Police Department. Non-emergency calls and inquiries may be made to the Warren Animal Shelter during normal business hours. The Warren Animal Shelter impounds stray dogs and sick or injured cats ONLY. Persons wishing to surrender an animal must contact a private humane agency (ie: RISPCA, Providence Animal Rescue League, Potter League, etc.)

Richmond Emergency Management Agency implements policies related to hazard mitigation and emergency management programs and activities. The EMA also has general supervision of all civil preparedness duties.

Affordable Housing 62+

Grassroots recovery community organization of individuals supporting those in recovery and those seeking recovery with peer support/referrals/social events. Dedicated to developing a voice of empowerment for, and decreasing the stigma about, alcohol and other drug dependency recovery. RICARES works with various agencies, in all sectors of business; private, public, and for profit; to conduct focused outreach. Their goal is establish the presence of people in long term recovery at all levels of the community.

The Palliative Care program provides home health services to patients with serious and progressive health conditions who want to continue active treatment, seeking a cure or remission. Patients in this program often have life-threatening conditions such as: Cancer; Cardio-pulmonary disease; Lung or kidney disease; Neurological diseases such as Parkinson or Alzheimer; Other serious progressive diseases. Patients in this program may continue any level of active treatments for their diseases in the comfort of their home. The Visiting Nurse Home & Hospice supports patients and their families in the following ways: Manage complex pain and symptoms; Manage medications; Coordinate care with other health care professionals; Provide support for treatment decisions; Ease stresses and anxiety for patients and families during life-threatening disease.

Southlawn Palms is a subsidized, project-based, 65 unit apartment community with 1-3 bedroom units.

Facilities which house a collection of materials including books, manuscripts, journals, government documents and nonprint formats such as paintings, musical recordings, videotapes, films and data files on magnetic tape which are organized to provide physical, bibliographic and intellectual access to a targeted group of people and which generally make available staff to provide services and programs related to the information needs of the targeted group.

Services: Educational and advocacy programs on urban environmental issues and stewardship of open spaces. Activities focus in four areas: schools, neighborhood organizations, business and industry and municipal operations. The Aldersbridge Physical Therapy & Wellness Center is housed in an 1,800 sq. ft. state-of-the-art suite within Linn Health & Rehabilitation located at 30 Alexander Avenue in East Providence, R.I. It’s the largest in-house rehabilitation unit of its kind within a nonprofit skilled nursing facility in the East Bay, and is open to residents and the public on an outpatient basis. The Medicare Prescription Drug program gives you a choice of prescription plans that offer various types of coverage. You may be able to get extra help to pay for the monthly premiums, annual deductibles, and co-payments related to the Medicare Prescription Drug program. However, you must be enrolled in a Medicare Prescription Drug plan to get this extra help.

The Victims of Crime Act Program (VOCA) provides advocacy and support services to all identifying as past or current, reported or unreported, victims of crime. VOCA assesses victims’ situations and helps identify next steps. The program links you with resources to provide you with on-going or short-term support to address any needs related to having survived a crime. VOCA can help access the criminal justice system if appropriate and victims’ compensation fund if eligible.

Home services include skilled nursing, physical, occupational, and speech therapies, as well as nursing assistance with the activities of daily living.
