After an initial intake screening you may be referred to an on-site psychiatric practitioner for further evaluation and treatment. Individualized treatment and recovery plans might include medication management supervised by the psychiatric practitioner and ongoing co-occurring counseling with clinical staff. CODAC’s newest site at Eleanor Slater Hospital offers a program for patients with Opioid Use Disorder (OUD) that has the ability to utilize alternative therapies in managing chronic pain. Assisted Daily Living, Inc. provides clients with a wide array of services designed to ensure superior patient care in the familiar comfort of home, often providing a happier and more affordable alternative to nursing, or assisted living facilities. Services can be arranged for a few hours a week or around the clock. ADL begins by reviewing the specific needs of the client through discussions with family members, as well as the care recipient's doctor, social workers or therapists. A customized program of care is developed to manage the client's daily needs. ADL will match the clients needs to the best possible caregiver, based on both professional experience and on ability characteristics. Basic Health Monitoring: Skin - monitor for signs of breakdown, apply pressure reduction measures; Nutrition - monitor weight, nutritional intake, adhere to dietary restrictions; Vital Signs - take blood pressure, temperatures, pulses and respirations. Report abnormalities to physician; Intake / Output - encourage adequate fluids, monitor fluid intake and urine output; Lab Monitoring - perform prescribed blood work and report results to physician; Diet - prepare special prescribed diets, encourage compliance with restrictions; Glucose Monitoring - check blood glucose per orders, report results to physician; Insulin - teach administration of insulin per prescribed regime, follow-up and report compliance to physician. Diabetes: New Diabetic - teach special skin care, insulin injection technique, blood glucose monitoring, hypo/hyperglycemia and related emergency protocol. Medication Management: When capable - remind to take medications and follow-up; When Forgetful - give oral medications prepared by responsible party and follow-up; Supplies - manage supply of prescriptions to have all meds on-hand; Teaching - instruct new medications or monitor changes in regime per plan of care, assess response, side effects and adverse reactions. Catheters (Foley Indwelling and External): New Catheter - teach drainage bag changes and cleaning, securing tubing, trouble shooting dislodged catheters placement, blockages, drainage amounts; Ostomies - Colostomy, Urostomy Changes - insert catheter per authorized plan of care and as needed; New Ostomy - teach ostomy care, preventive skin care, troubleshooting; Care - assist with ostomy changes, monitoring peri-stomal skin. Wound Care: Wound / Bedsore - assess wound, incision or bedsore healing, signs and symptoms of infection and evaluate response to treatment; Teach Care - instruct wound or bedsore regime, evaluate technique, compliance; Treatment - perform wound / bedsore care per prescribed regime, report signs and symptoms infection and progress to physician. Private Duty Nurses: Available to care for patient's in Hospital's & Nursing Homes. Home Assessment: evaluate home safety needs, arrange for adaptive equipment. Therapy: assess, plan, perform and evaluate physical therapy to regain strength and maximal activities of daily living plan. Teaching: instruct prescribe exercise regime and safe ambulation with/without assistive devices. Assistance: help perform prescribed exercises, report progress to physician. The Ocean Tides School Providence Campus is an alternative day school for students in grades seven through twelve. Offering a full high school Rhode Island Regents diploma, the school is licensed by the Rhode Island Department of Education (RIDE) and accredited by the New England Association of Schools and Colleges (NEASC). Additionally, the Providence Campus offers a certified special education program and a career/technical education program. The Rhode Island Veterans Home is a 110-acre complex on Mount Hope Bay in Bristol. The home provides quality nursing and residential care to Rhode Island war veterans. Some of the services provided include social, medical, nursing and rehabilitative services. Services Approximately 208 beds. Assisted living and nursing care, including secure units (Alzheimer's care) and palliative care. X-ray, dental, and pharmacy services, physical therapy and occupational therapy, psych services, transportation to PVAMC. Rehabilitation Services includes physical and occupational therapy. Social Services includes individual and group therapy and specialized support groups. Transitional Services include short-term stays in dorms or a more intensive Transitional Supportive Program for homeless veterans. Activities and Services for Residents include movies, crafts, card games, bingo, birthday parties, cookouts and day trips. Religious Services are also available. In addition to the Veterans Home buildings, the facilities include a library, gift shop, cafe, bank, barbershop, and beautician. The Veterans Home has an active Family Council that is open to family members. They meet monthly to discuss common issues and concerns affecting the residents of the home.

The Gately Building offers 13 affordable apartments, a large commercial space with a complete commercial kitchen, and a large community space. PCF Development has partnered with Farm Fresh Rhode Island to turn the Gately Building’s first commercial space into a permanent location for job training and retail operations. Farm Fresh Rhode Island is a non-profit food incubator that strives to bring food producers, consumers and markets together.
