Home Care We all need a little help sometimes. Home care services help people age strong in the community, connecting them with in-home supports, adult day health programming, and/or assisted living services. For older adults with care needs, the @Home Cost-Share program is available to help with some care expenses. Eligible participants can access in-home services – such as help with housekeeping, meal preparation or personal care – and/or community-based programming at adult day health centers at a reduced rate. You may be eligible for the program if you are age 65 or older, meet income guidelines, and need some assistance in the home with your personal or health care. The Unit provides a safe, structured environment that meets physical, emotional, social, and spiritual needs throughout the disease progression. An emphasis is placed on residents succeeding, dignity, and support of family members. Short Term Rehabilitative Care Short Term Rehabilitative Care is designed for individuals in need of rehabilitation following hip or knee replacement, heart attack, stroke, or other acute medical event. Our goal is to rehabilitate individuals to restore their previous level of functioning with the goal of returning back to their home. Skilled nursing services; on-site physical, occupational, and speech therapies are available. Short term stays range from one week to several weeks, depending upon the individual’s medical condition. Hospice Suite The Hospice Suite provides care for terminally ill individuals who qualify for general inpatient level of care. The Suite affords residents a lovely homelike environment with ample room for visiting family members.

Intensive Outpatient Program The Intensive Outpatient Program (IOP) at CODAC is an ideal opportunity for those individuals who find they have a need for increased access to support and treatment. IOP is approximately 8 weeks, and consists of individual and group sessions totaling a minimum 9 hours per week. IOPs may be utilized at any point along the treatment or recovery cycle and based on level of therapeutic need. If you are interested in this level of care speak with your CODAC provider. General Outpatient Services General Outpatient Programs are for individuals interested in treatment for Substance Use Disorder symptoms. General Outpatient programs utilize scheduled individual and group counseling sessions and drug/alcohol testing to assist patients in identifying and maintaining healthy life choices. CODAC counselors also work with the patient to integrate recovery support services in their treatment. We will to continue working with patients who have met their initial goals and want to maintain and strengthen their recovery. CODAC counselors work closely with their patients to achieve individual goals and/or meet the requirements of the referring authority. Most importantly, our programs are designed to assist individuals to develop and integrate a recovery oriented lifestyle that includes family, friends, health, wellness, joy and productivity. All CODAC programs emphasize the value recovery oriented lifestyle benefits and encouraged strongly encourage the integration of family members into the treatment and recovery process.
