Under this model, the child comes to a child-friendly environment and meets with a trained interviewer while the other team members observe from another room via a video screen. The child is informed of the equipment and of the observing team, and is never forced to answer any questions.

First Connections at Visiting Nurse Home & Hospice provides free, voluntary, confidential home visits in to pregnant women and families with young children. Conduct home visits: Trained nurses, social workers, and community health workers meet with families in their homes to talk about their needs, answer questions they may have, and conduct comprehensive assessments. If needed, home visitors can make arrangements for interpretation services during home visits. If you are an expecting parent, you may be contacted after the birth of your child to arrange a home visit.
Engage families who may benefit from home visits. Link families with medical homes and social services: Home visits are based on the needs of the family. Visit topics may include health education and connections with appropriate healthcare services, human services, and community resources. If a child does not have a medical home, VNHH can help the family find one. Visiting Nurse Home & Hospice can also conduct child wellness screenings and communicate the results with the child’s healthcare provider. Centers for Independent Living teach a wide range of skills needed to live independently and manage life with a disability. Skills training is available to persons with disabilities in almost any topic where there is a need for more independence. CILs work with individuals to increase their skills which can help them gain more control of their lives.
Skills training can include basic everyday responsibilities such as: self-care, meal preparation, budgeting, and arranging transportation. It also can address housing needs, teaching individuals the importance of self-advocacy, one’s need for accommodations and/or rights in the community.
***Life skills training is goal directed and does not involve long-term case management or therapeutic intervention.
