The East Bay Senior Medicare Patrol (SMP) Program helps Medicare and Medicaid beneficiaries prevent, detect, and report health care fraud. SMP not only protects seniors, it also helps to preserve the integrity of the Medicare and Medicaid programs. Senior Medicare Patrol staff and volunteers work with beneficiaries to: -Review their Medicare Summary Notices for accuracy -Protect their identity -Identify and report errors on their health care bills -Identify deceptive health care practices, such as illegal marketing, providing unnecessary or inappropriate services, and charging for services that were not provided -SMP also conducts informational sessions for seniors about avoiding Medicare and Medicaid scams.

WARM provides five separate, individual rooms of long-term transitional housing. This program, known as WARM-Up, occupies the same building – but not the same floor – as the shelter. Participants in this program may stay up to two years. WARM Supportive Homes provides permanent housing to six disabled individuals and two families with disabled adult members. We provide a safe, supportive environment, and thorough case management.

Washington County Community Development Corporation provides decent affordable housing for residents of Washington County. Programs: Home Repair Program - WC CDC manages the popular Home Repair program for all towns in Washington County, as well as West Greenwich and Exeter. This program provides grants and loans to income eligible home owners. These grants and loans are intended to ensure folks remain in their homes by performing reasonable and necessary repairs. We focus on code violations, health and safety issues, and any other repairs needed to maintain a healthy safe and sanitary dwelling. Help with roof, window, electric, plumbing and structural repairs that are necessary for your health and safety. Septic and well replacement programs are also available. Housing Preservation Grants - WC CDC provides grants awarded through USDA Rural Development. These grants supplement the CDBG Home Repair Program in rural towns in Washington County. CDBG Management - WC CDC manages the Community Development Block Grants for 9 Towns. These grants are utilized by non profits throughout the region to provide services and /or improve public facilities. WC CDC services include: Applications, State Contracts, Environmental Reviews, Sub recipient contracts, Payments, Reporting and Documentation, Affordable Housing Development. WC CDC has developed 124 units of affordable housing since 2005, by either partnering with another non profit or on our own. Provides free, voluntary, confidential home visits to pregnant women and families with young children, birth to age three. Trained nurses, social workers, and community health workers meet with families in their homes to talk about their needs, answer questions they may have, and conduct comprehensive assessments. Home visits are based on the needs of the family. Visit topics may include health education and connections with appropriate healthcare services, human services, and community resources. If a child does not have a medical home, we can help the family find one.
