Eastgate Nursing & Rehabilitation Center joined the Health Concepts family of caregivers in 2012. Since that time, the expert caregivers in our 68-bed skilled nursing center have helped hundreds of people live their best life possible.

Shared Living supports adults with disabilities and seniors living in the community who are unable to live alone independently. Participants may live either with caregivers who are family members or non-relatives. Person-centered support is provided by a team of professionals, coordinated through Case Managers and Registered Nurses during regular home visits, specialized trainings, and 24-hour support.

Hospice care is designed to comfort patients and family members when patients have reached a point in their lives where the priority of care turns to ensuring the patient is at ease, rather than curing the illness.

Urgent Care

Pawtuxet Valley Urgent Care is a medical practice providing prompt treatment of all non-life-threatening acute injuries and illnesses for patients of all ages on a walk-in basis. Providers include Physicians, Physician Assistants and Family Nurse Practitioners. Areas of expertise include: Cuts, sprains, and strains; Minor traumas; Cold and Flu; Headache; School and sports physicals; PPD and Vaccine titer testing; Infections; Vaccines (including Flu, Pneumonia, Hepatitis). On-site x-ray and lab services.

Services: Long term residence for women who are homeless due to battering or addiction. We offer a variety of services including counseling, education, life skills, etc.

The Cranston Fire Department manages the Emergency Management Agency. The Cranston EMA is responsible for emergency planning, operating shelters, planning evacuation routes and managing the Community Emergency Response Team (CERT).
