Heritage Village includes three buildings for seniors and six for families. All of the apartments have Section 8 rental assistance which allows each household to predictably pay 30% of its income for rent because the rental subsidies are ‘project-based’ meaning they accompany the property rather than a current resident guaranteeing the subsidy’s availability for future households. Services: Thirteen senior meal sites in northern Rhode Island serve a weekday lunch geared to the nutrition requirements of people over 60 (reservations required). Respite Care

One of four community-based health centers offered by CCAP. Provides a full range of primary health and behavioral health care services including OB-GYN, Pediatrics, Geriatrics, maternal child home visiting, HIV Testing, Case Management, Mental Health Counseling/Substance Treatment including MAT and Oral healthcare, as well as eligibility assessments for state-funded programs.

Services: Provides educational and training information for post secondary education, financial aid and admissions information, application assistance in plus career counseling and assessment, referral to other social services and advocacy. Day Program - a service program designed to respond to the needs of individuals with developmental disabilities including autism behavioral and emotional disabilities. Access to vocational, social and communications skill programs allow clients to reach their greatest level of independence, productivity, empowerment and community access. Stepping Out a non-profit organization formed to provide social, educational, and cultural activities for young adults with special needs. Residential Program - provides a variety of residential accommodations for adults with developmental disabilities and special needs. To respond to the needs of adults with developmental disabilities, autism, emotional disorders and other challenging behaviors to help them reach their greatest level of independence, productivity and integration. Glenark Landing Apartments is 89 apartments of affordable units, that is, geared to people earning no more than 60 percent of area median income. There are one, two, and three bedroom apartments to accommodate different family sizes; a laundry room; and a Community Room and Kitchen that all residents can enjoy.

Avatar is a certified Placement Agency for Shared Living programs throughout Rhode Island. Avatar Residential was one of the first agencies in Rhode Island to provide Shared Living Arrangements (SLAs) as an alternative to group home living. The key to success is a strong relationship between residents and Shared Living Providers. Avatar coordinates 27 SLAs throughout the state of RI. Some of the people that Avatar supports have found a good fit in their apartment/independent living programs. They tailor services to what people need (shopping, budgeting, recreation) while also fostering a great deal of independence and self-sufficiency. Avatar also provides residential services to people who live at home with their families.
