Eligible participants can access in-home services – such as help with housekeeping, meal preparation or personal care – and/or community-based programming at adult day health centers at a reduced rate. You may be eligible for the program if you are age 65 or older, meet income guidelines, and need some assistance in the home with your personal or health care. CSP participants have received a qualifying diagnosis of a psychotic disorder, mood disorder, obsessive compulsive disorder and have had difficulties engaging with treatment.This program enables individuals to maintain psychiatric stability, increase their quality life in the community, and ultimately achieve their own personal recovery. Qualifying clients will have access to psychiatric services, counseling, case management services, education, peer support, residential services, and transportation to and from medical appointments.
Thrive's Community Support Program is comprised of three integrated support teams including the Assertive Community Treatment (ACT) Team and Integrated Health Home (IHH) Teams. An Outpatient level of care is available to those who have similar needs but require less intensive care.
