PACE Organization of Rhode Island is a not-for-profit health plan that offers services for qualifying adults age 55 and older. These older adults all have complex health needs but want to remain at home. As the insurer, provider, and coordinator of health care and wellness, PACE-RI tailors care plans to meet each participant’s individual needs through a range of service offerings. These services can be delivered in the home, at our health clinics and day centers, and beyond, providing great peace of mind for participants and their caregivers. Since 2005, PACE-RI has served more than 2,000 participants and their families across Rhode Island. The MME counselors provide person-centered options counseling to Medicare-Medicaid individuals to ensure that they receive clear, unbiased, and consumer friendly information and one-on-one counseling to help them make informed decisions about options they have for receiving their Medicare and Medicaid benefits. They also help with the enrollment or dis-enrollment process as needed.

One of the core functions of the Ocean State Center for Independent Living is assisting people with disabilities to move out of nursing homes and other institutions into the independent living arrangement of their choice, as well as helping to prevent people with disabilities from entering these institutions. The Ocean State Center for Independent Living (OSCIL), a non-residential, non-profit organization, can help seniors and adults with disabilities who are interested in transitioning from nursing facilities back into the community—either moving back to their own home or apartment or to the home of a relative or friend. OSCIL’s Community Living Option Program has been assisting persons with disabilities to move from a nursing home setting to a less restrictive environment for many years. OSCIL staff is able to work with each individual to identify their specific needs, help secure appropriate housing, establish community connections and obtain support services—all necessary for a safe and sustainable transition. Home Care We all need a little help sometimes. Home care services help people age strong in the community, connecting them with in-home supports, adult day health programming, and/or assisted living services. For older adults with care needs, the @Home Cost-Share program is available to help with some care expenses. Eligible participants can access in-home services – such as help with housekeeping, meal preparation or personal care – and/or community-based programming at adult day health centers at a reduced rate. You may be eligible for the program if you are age 65 or older, meet income guidelines, and need some assistance in the home with your personal or health care.
