Our mission is �to provide optimal care and services to individuals confronting dementia, and to their caregivers and families�through member organizations dedicated to improving quality of life.� AFA and/or our member organizations offer all or most of the following services: Licensed/certified social workers. Professionals offer vital information and counseling to those affected by Alzheimer's disease and related dementias. Resource and referral centers. Knowledgeable staff provides information and links to community resources, healthcare professionals, social services and other support. Educational conferences and workshops. They are presented in English as well as in other languages to reach ethnic populations. Identification programs. Bracelets and state-of-the-art technology, such as Project Lifesaver's wristband transmitters, help with the identification and safe return of wanderers. Early-stage and middle-stage programs. These include social and intellectual stimulation programs and support groups for individuals in the early and middle stages of dementia. Social-model adult day services. These programs provide cognitive stimulation and recreational therapies to individuals in various stages of the disease. Family-oriented services. Support groups and bereavement groups are available for caregivers and other family members. Respite programs. In-home respite care and overnight respite programs are designed to provide temporary relief for family caregivers. Telephone "buddy" programs. Staff or volunteers, many of whom have been caregivers themselves, support and educate caregivers. Caregivers can call into a telephone support network, Care Connection, each week. Memory screenings. These free, confidential screenings are administered to those concerned about memory loss, with the goal of early detection. Books, DVDs, products and equipment. Hospice is the most appropriate medical treatment option when an individual has decided to discontinue medical treatment and focus on comfort and quality of life. Hospice encompasses the needs of the individual's family or caregivers so that together, they are able to fully enjoy meaningful time together. Providing care at the place the patient calls home is the basic philosophy of hospice, whether it is at home, in a family member's home, a nursing home, an assisted living, or a residential care facility.

Assisted Daily Living, Inc. provides clients with a wide array of services designed to ensure superior patient care in the familiar comfort of home, often providing a happier and more affordable alternative to nursing, or assisted living facilities. Services can be arranged for a few hours a week or around the clock. ADL begins by reviewing the specific needs of the client through discussions with family members, as well as the care recipient's doctor, social workers or therapists. A customized program of care is developed to manage the client's daily needs. ADL will match the clients needs to the best possible caregiver, based on both professional experience and on ability characteristics. Basic Health Monitoring: Skin - monitor for signs of breakdown, apply pressure reduction measures; Nutrition - monitor weight, nutritional intake, adhere to dietary restrictions; Vital Signs - take blood pressure, temperatures, pulses and respirations. Report abnormalities to physician; Intake / Output - encourage adequate fluids, monitor fluid intake and urine output; Lab Monitoring - perform prescribed blood work and report results to physician; Diet - prepare special prescribed diets, encourage compliance with restrictions; Glucose Monitoring - check blood glucose per orders, report results to physician; Insulin - teach administration of insulin per prescribed regime, follow-up and report compliance to physician. Diabetes: New Diabetic - teach special skin care, insulin injection technique, blood glucose monitoring, hypo/hyperglycemia and related emergency protocol. Medication Management: When capable - remind to take medications and follow-up; When Forgetful - give oral medications prepared by responsible party and follow-up; Supplies - manage supply of prescriptions to have all meds on-hand; Teaching - instruct new medications or monitor changes in regime per plan of care, assess response, side effects and adverse reactions. Catheters (Foley Indwelling and External): New Catheter - teach drainage bag changes and cleaning, securing tubing, trouble shooting dislodged catheters placement, blockages, drainage amounts; Ostomies - Colostomy, Urostomy Changes - insert catheter per authorized plan of care and as needed; New Ostomy - teach ostomy care, preventive skin care, troubleshooting; Care - assist with ostomy changes, monitoring peri-stomal skin. Wound Care: Wound / Bedsore - assess wound, incision or bedsore healing, signs and symptoms of infection and evaluate response to treatment; Teach Care - instruct wound or bedsore regime, evaluate technique, compliance; Treatment - perform wound / bedsore care per prescribed regime, report signs and symptoms infection and progress to physician. Private Duty Nurses: Available to care for patient's in Hospital's & Nursing Homes. Home Assessment: evaluate home safety needs, arrange for adaptive equipment. Therapy: assess, plan, perform and evaluate physical therapy to regain strength and maximal activities of daily living plan. Teaching: instruct prescribe exercise regime and safe ambulation with/without assistive devices. Assistance: help perform prescribed exercises, report progress to physician.
