Our mission is �to provide optimal care and services to individuals confronting dementia, and to their caregivers and families�through member organizations dedicated to improving quality of life.� AFA and/or our member organizations offer all or most of the following services: Licensed/certified social workers. Professionals offer vital information and counseling to those affected by Alzheimer's disease and related dementias. Resource and referral centers. Knowledgeable staff provides information and links to community resources, healthcare professionals, social services and other support. Educational conferences and workshops. They are presented in English as well as in other languages to reach ethnic populations. Identification programs. Bracelets and state-of-the-art technology, such as Project Lifesaver's wristband transmitters, help with the identification and safe return of wanderers. Early-stage and middle-stage programs. These include social and intellectual stimulation programs and support groups for individuals in the early and middle stages of dementia. Social-model adult day services. These programs provide cognitive stimulation and recreational therapies to individuals in various stages of the disease. Family-oriented services. Support groups and bereavement groups are available for caregivers and other family members. Respite programs. In-home respite care and overnight respite programs are designed to provide temporary relief for family caregivers. Telephone "buddy" programs. Staff or volunteers, many of whom have been caregivers themselves, support and educate caregivers. Caregivers can call into a telephone support network, Care Connection, each week. Memory screenings. These free, confidential screenings are administered to those concerned about memory loss, with the goal of early detection. Books, DVDs, products and equipment. These are made available to educate families and healthcare professionals, and to meet essential care needs and improve quality of life for those with dementia.

A multi-disciplinary clinic intended to provide continuing supervision for all medical/social aspects of the patient with Hemophilia. Team includes a Nurse Coordinator, Social Worker, Dentist, Orthopedic Surgeon, Physical Therapist and Hematologist.

A comprehensive, State licensed adult day care program to provide medical, social and rehabilitative services to chronically impaired elders and disabled adults. Services include: nursing supervision, personal care, counseling, caregivers support/respite, exercises, activities, specialized therapies and nutrition. The Adult Day Services Program provides an array of supportive services designed to upgrade and increase the activities of daily living for older persons dependent upon continual family support and supervision. The major goal of the program is to reduce the potential for institutionalization of dependent elderly individuals otherwise at risk, and to increase their quality of life through the enhancement of their activities of daily living. The program provides supervised supportive care for frail elderly persons to meet the needs of the functionally impaired individual. Heatherwood Rehabilitation & Health Care Center offers short-term rehabilitation, long-term, respite, hospice and dementia care.

Programs that offer any of a variety of therapeutic approaches which are intended to maximize the existing cognitive functioning of people who have Alzheimer's disease, Pick's disease or other forms of cognitive impairment. Interventions include structured activities and exercises that are designed for people who have a short attention span and require extra verbal and visual stimulation with the objective of enhancing the individual's perception of the environment, promoting trust, reducing anxiety, avoiding overstimulation and maximizing communication skills.

Cedar Crest Subacute and Rehabilitation programs, a 156 bed medical center, were designed for patients with physical, medical and cognitive impairments who would benefit from patient-centered care and an interdisciplinary team approach. Our goal is to work with patients to achieve their optimum level of functioning. When those treatment goals are met, the patient is ready to move on. Treatment plans can include physical therapy, occupational therapy and speech-language therapy, along with nursing care oriented to the needs of rehabilitation patients. The patients most appropriate for this program are those whose diagnoses are orthopedic conditions, stroke, neurological disorders, or those who have experienced general weakness or de-conditioning following surgery or an illness. Dementia and Alzheimer's Services provides compassionate support for those who are caring for a loved one with dementia. They provide information, education and support for individuals with these diseases, along with the families and professionals who care for them.

The Rhode Island Veterans Home is a 110-acre complex on Mount Hope Bay in Bristol. The home provides quality nursing and residential care to Rhode Island war veterans. Some of the services provided include social, medical, nursing and rehabilitative services. Services Approximately 208 beds. Assisted living and nursing care, including secure units (Alzheimer's care) and palliative care. X-ray, dental, and pharmacy services, physical therapy and occupational therapy, psych services, transportation to PVAMC. Rehabilitation Services includes physical and occupational therapy. Social Services includes individual and group therapy and specialized support groups. Transitional Services include short-term stays in dorms or a more intensive Transitional Supportive Program for homeless veterans. Activities and Services for Residents include movies, crafts, card games, bingo, birthday parties, cookouts and day trips. Religious Services are also available. In addition to the Veterans Home buildings, the facilities include a library, gift shop, cafe, bank, barbershop, and beautician. The Veterans Home has an active Family Council that is open to family members. They meet monthly to discuss common issues and concerns affecting the residents of the home.

South Kingstown Nursing & Rehabilitation Center offers licensed rehabilitation and subacute unit and secure Alzheimer’s care as well as long-term care for those in need of assistance with chronic illness or multiple activities of daily living. Services: Provides therapeutic day services to victims of Alzheimer's Disease or a related dementia as well as education, support and advocacy for family caregivers. The Center is a safe, secure and happy "home away from home" where participants are encouraged to be optimally independent, but where assistance is always close at hand. Services include but are not limited to multidisciplinary care planning and case management, information and referral, nursing and personal care, nutritional services, therapeutic activities, and assistance in accessing transportation, financial assistance, and other health and enabling services. Through collaborative, contractual and cooperative agreements, the Center is able to enhance core services with physical, occupational and speech therapies; hospice care; podiatry; hair care and a variety of special programs. The staff team includes registered and licensed practical nurses, case managers, certified nursing assistants and activity professionals who create individualized plans of care for each client.

Pine Grove Health Center is a Health Concepts Nursing and Rehabilitation Facility. A state-of-the-art skilled nursing and sub-acute rehabilitation center, Pine Grove Health Center has been an important part of the path from hospital to home for thousands of people looking to recover or rehabilitate as quickly and comfortably as possible from accident or illness. The interdisciplinary care team also helps manage chronic, long term conditions like respiratory illness or multiple sclerosis, in a setting that mirrors the comforts of home. Our specialized ventilator unit is one of only two units of its kind in Rhode Island.
