MLK Jr. Community Center is a nonprofit service provider for at-risk individuals, families and seniors. Hunger Services On Site Food Pantry - providing non-perishable food, frozen meat, fresh produce, bread and household and personal care items to our Newport County residents. All clients are eligible to shop once per month. Mobile Food Pantry - visits neighborhoods across Newport County each month on a rotating schedule. The mission of the Woonsocket Senior Center is to improve quality of life and to maintain individuals in their preferred home environment in the community through nutrition programs, Health & Wellness Education, Adult Day Program, and Information and Referral. Senior Services, Inc. has provided services to seniors and adults with disabilities residing in Northern Rhode Island since 1975. In accordance with the Congregate Meal Program financed through a grant with R.I. Office of Healthy Aging, Senior Services, Inc. provides nutritionally balanced lunches five days a week at meal sites throughout northern R.I. Senior Services, Inc. offers activities, entertainment, computer classes, and Wellness programs, including our weekly blood pressure clinic and exercise classes. Senior Services, Inc. also offers an Adult Day Program ("A Home for the Day"), and a Community Information Specialist who can provide information on eligibility for all state and local programs.

The Picillo Center is a community center where older individuals can participate in community-based outings, physical fitness exercises, art therapy and horticulture activities throughout the day. The gardening project, initially started in 2009, has transformed into a genuine Horticulture program and business. Since its inception, this small garden has grown into a larger fenced garden, incorporating raised flowerbeds allowing for ease with plant care. With interest and ability improving, employees of the program successfully sponsored a number of fundraising events for the purpose of purchasing a greenhouse. With this in place, gardeners are able to begin their harvest earlier by starting with the seedlings. While both of these projects are enjoyable and healing, they also serve as an employment opportunity with produce and flowers being sold to individuals, local vendors and at farm fresh fairs.

Saint Elizabeth Adult Day Centers helps families keep their loved ones at home by providing a wide array of supportive services during the day within a secure and nurturing environment.

Their experienced case management professionals will assess the client and living conditions to make improvements to overall quality of life. Collaborative care plans are developed, services implemented and continued evaluations and monitoring are provided to ensure success.

PACE - Program for All-Inclusive Care for the Elderly. Provides comprehensive health care services and supports to older adults who live at home or in another community setting and eligible for Medicaid LTSS.

Rhode Island's Statewide Self-Advocacy organization. Works to help people understand information more clearly, learn about rights, participate in their communities and share their unique gifts with the rest of society. Also works to raise public awareness about disabilities. Activities include:

• Hosting an annual statewide Self-Advocacy

conference. Approximately 430 people attend.

• Hosting an annual Statewide Self-Advocacy Town Hall style meeting. Approximately 300 people attend.
• Designing/Adapting accessible materials in a variety of formats to help people understand issues related to Self-Advocacy.
• Coordinating a year-long Leadership Series training for individuals who have a Developmental Disability, families and direct support staff.
• Bringing disability awareness experiences to schools, colleges, businesses, support agencies and others.

Fox Point Senior Center works with: Meals on Wheels, URI School of Pharmacy, URI School of Nursing, Department of Elderly Affairs, The City of Providence, SNAP, The Fox Point Boys' and Girls' Clun, United Way 211, and senior employment services. Their mission is to provide seniors and individuals with disabilities a daily hot meal, health and wellness programs, information and referral programs, translation services and individual attention as needed.

Services: Both an insurer and a provider, provides all aspects of health care to its enrollees. Offers primary and specialty physician services, rehab and home care. Also, hospital and drug coverage as well as a fully functioning, accessible day center with transportation. Provided day care center services, prescriptions, nutrition, social work life enrichment behavioral health.

The mission of the Tiverton Senior Center is to reach out to all citizens in the community who are 55 or older to offer socialization, education, recreation, congregate meals, and various health and informational services that will improve the quality of life for all participants.

PACE Organization of Rhode Island is a not-for-profit health plan that offers services for qualifying adults age 55 and older. These older adults all have complex health needs but want to remain at home. As the insurer, provider, and coordinator of health care and wellness, PACE-RI tailors care plans to meet each participant’s individual needs through a range of service offerings. These services can be delivered in the home, at our health clinics and day centers, and beyond, providing great peace of mind for participants and their caregivers. Since 2005, PACE-RI has served more than 2,000 participants and their families across Rhode Island. The MME counselors provide person-centered options counseling to Medicare-Medicaid individuals to ensure that they receive clear, unbiased, and consumer friendly information and one-on-one counseling to help them make informed decisions about options they have for receiving their Medicare and Medicaid benefits. They also help with the enrollment or dis-enrollment process as needed.
