Community Care Alliance strives to empower people experiencing economic insecurity, mental illness and addiction, housing issues, and trauma-related concerns. They provide clinical and support services for individuals and families with long-term needs associated with mental illness and substance abuse. Their goal is to help people live as independently and as fully integrated within the community as possible These might include basic needs assistance to address emergencies— counseling, treatment, support services and residential programs for individuals and families impacted by serious mental illness and addiction— emergency and supportive housing to prevent homelessness and promote stabilization— alternative education and services for families with young children and adolescents— and employment, financial literacy and vocational training to improve self-sufficiency.

For those who have experienced the recent death of a loved one, Visiting Nurse Home & Hospice provides several Bereavement Support Groups throughout the year. These groups address a variety of topics designed to help, heal and support those who have lost a loved one. These groups are open to the public and free of charge.

In addition to 24 hour/seven days a week nursing care, Saint Antoine Residence offers the following special services: Special Care Unit The Special Care Unit is a 60-bed unit for residents with Alzheimer’s and related dementias. The Unit provides a safe, structured environment that meets physical, emotional, social, and spiritual needs throughout the disease progression. An emphasis is placed on residents succeeding, dignity, and support of family members. Short Term Rehabilitative Care Short Term Rehabilitative Care is designed for individuals in need of rehabilitation following hip or knee replacement, heart attack, stroke, or other acute medical event. Our goal is to rehabilitate individuals to restore their previous level of functioning with the goal of returning back to their home. Skilled nursing services; on-site physical, occupational, and speech therapies are available. Short term stays range from one week to several weeks, depending upon the individual’s medical condition. Hospice Suite The Hospice Suite provides care for terminally ill individuals who qualify for general inpatient level of care. The Suite affords residents a lovely homelike environment with ample room for visiting family members.

Hospice Care, also referred to as palliative care, aims to improve quality of life and the physical, emotional, social, and spiritual needs for families of residents living with a serious illness. Scalabrini Villa collaborates with many different Hospice agencies to provide a variety of choices. Provides specialized palliative care and end of life services affording comfort and support to residents who are no longer seeking active treatment and have a life expectancy of months or weeks. The focus of the Hospice team and the Villa staff is the physical and emotional support to both the resident and their family members. Members of your hospice team at Scalabrini Villa may include physicians, a hospice doctor or medical director, skilled nurses, home health aides, social workers, mental health providers, trained volunteers, clergy and with special authorization could include: physical, occupational and speech therapists.

Supportive care for terminally ill patients. Care is provided where you live—be it your private home, an assisted living community, a skilled nursing or long-term care facility, or another residential setting. You’ll receive regular visits from members of your care team. They’ll adjust their visits to accommodate any changes in your needs to ensure the best care.

Offers open-ended groups for children ages 3-19 and their families after the death of a loved one. The groups meet on alternating weekdays. There is no fee charged for participation in our programs.

Services: Families Reaching Into Each New Day. Friends Way provides free peer based support groups, under the direct supervision of mental health professionals, for children and teens who are mourning the death of a loved one, usually a parent or sibling. We also provide crisis intervention, education and training on the subject of children's grief and loss to schools, mental health professionals, clergy and the community at large.

Hospice care is provided to patients who have advanced diseases that are considered incurable. Hospice care is about living each day to its fullest and helps to make a patient’s last months and days as comfortable and fulfilling as possible…making every moment count. Care is provided in the familiar surroundings of home or home-like setting. Care can also be provided in a long-term care setting or a hospital. The Hospice team focuses on patients’ and their loved ones’ physical, emotional and spiritual comfort. The goal is for the patient and family to spend their remaining days with comfort and dignity. Hospice team visits regularly and is available to answer questions and provide support. The team helps prepare family and friends for the loss of a loved one, and help them deal with their grief through bereavement support groups.

A support group for adults whose loved ones have died by suicide. The group is comprised of spouses, siblings, and parents who have suffered this particular loss. Meetings are on Wednesdays, in eight week blocks, from 6:00PM - 7:30PM.

Recovery: St. Matthias Ministry This ministry is dedicated to reaching and supporting individuals struggling with drug and alcohol problems. We also provide help to families in need of guidance or support due to a loved one’s drug or alcohol problem. These problems can affect their everyday lives and also the lives of their families and loved ones. Rhode Island has one of the highest percentages of illicit drug, marijuana and alcohol use in the country. The Tomorrow Fund sponsors a bereavement aftercare program that is facilitated by Clinical Social Workers and includes a monthly support group for bereaved parents.

Self-help support group for anyone who has experienced the loss of a child. Programs that provide a full range of supportive services for terminally ill individuals who are in the final stages of their illnesses and for their families. Services may include medical care, pain and symptom management, home nurse visitation, case management, emotional and spiritual support, and bereavement services for the patient and members of the family. Hospice care may be provided at home, in a freestanding hospice facility, a hospice unit of a hospital or in long-term care facilities such as nursing homes. In order to qualify for Medicaid or Medicare reimbursement, an individual must have a life threatening illness and must be certified by their physician to have entered the last six months of life.
