West Shore Health Center joined the Health Concepts family of caregivers in 1999. A 145-bed skilled nursing and rehabilitation center, West Shore offers licensed subacute and secure Alzheimer’s care as well as long-term care for those in need of assistance with chronic illness or multiple activities of daily living.

Weatherization improves the energy efficiency of homes and apartments. The Weatherization crew checks for energy use, insulation, air leaks, and safety hazards. Then they perform cost-effective improvements, such as installing insulation, smoke detectors, or caulking. Appliance Management Program (AMP) AMP helps clients use less electricity and water. The program provides energy-saving lightbulbs, faucet aerators, and appliances. AMP is funded by National Grid. Heating System Replacement Program The Heating System Replacement Program repairs or replaces old, broken, or inefficient home heating systems. This program is available for owner-occupied buildings only.

Programs that offer opportunities for non-English-speaking and limited-English-speaking adults to learn listening, speaking, reading and writing skills with an emphasis on developing the level of communication competence that is essential for adults who are living in a setting in which English is the primary language. Also included are programs that provide English language instruction for younger people.

Vouchers for clothing, furniture, household items are available to our thrift store. Vouchers also available to baby closet, toiletry closet and pet food closet.

SHIP Counseling

Raising Hope, Inc. is a community-based non-profit organization located in Providence, Rhode Island; providing youth development activities after-school and out-of-school times to children and youth ages 7 to 25 years old from immigrant and low income families. Programs include homework assistance, mentoring, youth soccer, career skills development, arts and cultural programs.

Belleville House is a subsidized 62+ housing complex.

Developer of affordable housing. Lending Programs for the purchase, repair and/or refinancing of an existing home providing HUD approves foreclosure prevention counseling. Women, Infants and Children (WIC) Supplemental Feeding and Nutrition Program provides food vouchers, allowing them to purchase milk, eggs, cheese, juice and iron fortified cereals and formula. The WIC coordinator provides nutrition education and counseling to all program participants on a monthly basis.

Spring Villa Apartments is subsidized housing 62+, handicapped or disabled.

Bethany offers counseling services for individuals, couples, groups, and families (including children, adolescents, and adults). Therapists are clinically certified with documented excellence in their area(s) of expertise. You’ll work with an assigned counselor to develop your plan for treatment, ensuring your spiritual, emotional, and mental health goals are addressed in a timely and helpful manner.

Monitor reportable infectious diseases: Identify newly diagnosed cases reportable under Rhode Island law and analyze trends in newly diagnosed cases. Locate people who have been exposed to infectious diseases and contact them and guide them to action. Control disease outbreaks, plan public health responses to outbreaks of infectious disease and react swiftly to contain outbreaks with healthcare providers in the community.

Hunt House Museum is located off Route 114A near the Rhode Island and Massachusetts border on the banks of the Ten Mile river, this house/museum is a repository for articles of significance to the city.

The Fogarty Center provides a variety of Employment & Day Supports to many of the people from its residential and other community settings. The Fogarty Center offers individual Person-Centered services for job seekers with intellectual/developmental disabilities by offering a comprehensive progression of employment-related services. These services include; Discovery, Career Exploration, Job Trial/Assessment, Placement, Job Coaching and Monitoring. The Fogarty Center also supports adults by planning meaningful activities throughout the day, based on personal preference, within a variety of settings including day programs as well as locations within the community. Services: Referrals to HIV testing sites. On general web site, type HIV testing into search line Services: Monitors the incidence of AIDS and conducts medical surveillance to prevent its spread. Sponsors free, anonymous testing and counseling services at several area sites. Conducts various educational programs on AIDS. Sponsors free, confidential diagnostic and treatment services for other sexually transmitted diseases at area hospitals and health centers. Provides educational and counseling services to prevent the spread of sexually transmitted diseases.

Applicant has 60 days after receiving the notice of the decision to ask for any type of appeals. There are four levels of appeal: Reconsideration, hearing by an administrative law judge, Review by the Appeals Council and Federal Court review.
