To provide eligible individuals with visual impairments the opportunities and support that will enable them to become independent, active, self sustaining members of their community. Eligibility: To be eligible for services, you must be visually impaired, with acuity of 20/60 or less in the better eye with correction, or a field loss of twenty degrees or less in the better eye. Our goal is to rehabilitate individuals to restore their previous level of functioning with the goal of returning back to their home. Skilled nursing services; on-site physical, occupational, and speech therapies are available. Short term stays range from one week to several weeks, depending upon the individual’s medical condition. Hospice Suite The Hospice Suite provides care for terminally ill individuals who qualify for general inpatient level of care. Hospital charity care requirements cover those essential services necessary to diagnose, correct, or cure a medical condition that endangers life, causes pain and suffering, results in illness or infirmity, or causes a physical deformity or malfunction. Both inpatient and outpatient services are included. To receive full charity care, patients annual family income must be up to and including 200% of the Federal Poverty Level. For partial charity care, patients annual family income must be between 200% and 300% of the Federal Poverty Level may receive assistance based on a sliding scale. In addition to income, hospitals have the option to consider other assets. Also must be denied from Medicaid first depending on income.
