Fifty nine units of permanent and transitional housing located on five campuses throughout the state. A coalition of concerned citizens, dedicated to all veterans who are homeless or at risk of being homeless. Housing (emergency and permanent). Permanent housing in Johnston and West Warwick. OSDRI follows the “housing first model” providing permanent and transitional housing combined with a full range of supportive services designed to address the individual veteran’s needs which led to his or her state of homelessness. Supportive services include but are not limited to: intensive case management; coordination of mental health and substance abuse treatment; assistance with VA benefits and employment, training and job placement services. Our focus is to give veterans a “hand up…not a hand out.” Vocational/educational training program at 790 Providence Street, West Warwick. Annual three day program in September for veterans desiring assistance with legal, housing or counseling needs. Service for housing- Help facilitate a combination of permanent supportive, transitional, and recovery housing to low income and disabled veterans to meet their needs. They own and operates permanent and transitional housing facilities. Each housing program offers various levels of supportive services. Housing is available for females, males, and their families. Housing assistance- eligible veterans who are literally homeless receive assistance with security deposits, rent, moving expenses and emergency shelter. Those at risk of homelessness, may receive help with rent in arears and utilities.

Adult Day Health Care Program is for adults with chronic health conditions and medical or physical limitations. Provides a combination of social activities, health care and rehabilitation services, and personal care. Participants attend on a set schedule from two to five days a week based on availability.

The Center provides hearing testing for about 600 people each year as well as hearing aid assistance for several hundred clients. In addition, about 40 evaluations are conducted annually related to speech therapy needs and approximately 60 clients per year are seen for on-going therapeutic services. Services include (but are not limited to) diagnostic hearing tests, hearing aid evaluations and dispensation; aural rehabilitation; assessment of and treatment for voice, articulation, fluency, and language disorders; communication and cognitive therapy for adults with neurological disorders resulting from strokes or traumatic brain injuries; and programs to improve fluency for ESL clients.

Children’s Personal Assistance Services and Support (PASS): consumer-directed program designed to support children with special health care needs. PASS provides families with the opportunity to take the lead role in directing the services received by their child. The family-centered approach empowers families and individuals to foster independence, support family care giving and decision making, respect family choices and values, and build on individual strengths. Respite Program: a consumer-driven program designed to enhance the quality of life for children with special needs and their families. The provision of Respite enables families to receive support in the delivery of care for their children. Children’s Home-Based Therapeutic Services (HBTS): clinically directed, specialized health services that are designed to meet the individual needs of children diagnosed with moderate to severe physical, developmental, behavioral or emotional conditions. Addresses behavioral concerns and the development of functional, social and communication skills. The HBTS Team is comprised of the parent(s), a clinical supervisor, a home based therapist, a family service coordinator, and when indicated specialty (treatment consultants, speech therapy, physical therapy, and/or occupational therapy). Together, the HBTS Team develops an individualized treatment plan with measurable goals and objectives. Additionally, clinical support and training is provided on a continuous basis to the home based therapist and family.

CODAC East Bay, serving East Providence, Barrington, Warren, Pawtucket and surrounding areas, treats more than 350 patients weekly. Programs include medication-assisted treatment (MAT) for opioid use disorder (OUD), intensified treatment programs (IOP), general outpatient treatment (GO) for alcohol and other substance use, behavioral healthcare services, tobacco cessation programs, and services for gambling addiction. CODAC East Bay, along with all other CODAC locations, has been awarded the designation of Center of Excellence (COE) for the treatment of opioid use disorder. As a designated COE, they have met or exceeded the highest standards of care (as determined by RI state agencies), are able to provide expedited access to treatment, and offer at least two of the three FDA approved medications for the treatment of opioid use disorder (methadone, buprenorphine (SuboxoneTM, SubutexTM) and oral and depot naltrexone (VivitrolTM). CODAC sites are currently the only providers in RI authorized to dispense all three of these medications. As a COE, CODAC guarantees that treatment will begin within 48 hours of your first phone call. In addition to individual counseling, CODAC East Bay offers a rich and comprehensive selection of group counseling opportunities, including groups for pre- and post-partum women; early recovery patients; co-occurring patients; DUI referrals; tobacco cessation and problem gambling patients; anger management referrals; and groups for adolescents. Offerings change over time to reflect shifting patient needs and interests.

OSDRI manages a community supported food pantry and personal hygiene program for use by in-need veterans stocked with non perishables. The pantry is available for use by in-need veterans during normal business hours and is stocked with non-perishables. Veterans wanting to use the pantry must bring proof of veteran status before admittance. Personal Hygiene Program- Personal hygiene products, including soaps, body wash, deodorant, toothpaste, razors, and other items are stocked at the Food Pantry.

The substance abuse task force identifies and addressed the issue of substance abuse in the city and its mission is to develop, implement, and advocate for effective community-based prevention initiatives.

General Outpatient Programs are for individuals interested in treatment for Substance Use Disorder symptoms. General Outpatient programs utilize scheduled individual and group counseling sessions and drug/alcohol testing to assist patients.
