ADL begins by reviewing the specific needs of the client through discussions with family members, as well as the care recipient's doctor, social workers or therapists. A customized program of care is developed to manage the client's daily needs. ADL will match the clients needs to the best possible caregiver, based on both professional experience and on ability characteristics. Basic Health Monitoring: Skin - monitor for signs of breakdown, apply pressure reduction measures; Nutrition - monitor weight, nutritional intake, adhere to dietary restrictions; Vital Signs - take blood pressure, temperatures, pulses and respirations. Helping individuals who are disabled by physical injuries or disease to regain maximum function with activities of daily living is always a main priority. Home Assessment: evaluate home safety needs, arrange for adaptive equipment. Therapy: assess, plan, perform and evaluate physical therapy to regain strength and maximal activities of daily living plan. Teaching: instruct prescribe exercise regime and safe ambulation with/without assistive devices. Assistance: help perform prescribed exercises, report progress to physician. MSW: Social Workers assist in the evaluation of social, emotional and environment factors affecting the ill and disabled. They may provide family or individual counseling for those in need or a crisis situation. Medical Social Workers offer support and often help indentify and locate appropriate community resources: Home care services, adult daycare, respite, long term care planning, financial assistance and support groups.
