Long Term Services and Supports (LTSS) serves people who have disabilities or chronic care needs in the setting ideal for them and their families. Services may be episodic or on-going. Services may be provided in a person's home, the community (for example, shared living or assisted living), or institutional settings (for example, intermediate care facilities, hospitals, or nursing homes) to optimize their health and retain their independence. Networking Breakfasts – regularly scheduled networking breakfasts offer an opportunity to meet other professionals, share information and make valued connections; National Philanthropy Day – Every November, AFP-RI hosts Rhode Island’s premier event celebrating philanthropy with nearly 500 people attending; Conference – The Chapter hosts a fundraising conference including educational tracks for all levels of experience, nationally recognized keynote speakers and great networking with other professionals. The major goal of the program is to reduce the potential for institutionalization of dependent elderly individuals otherwise at risk, and to increase their quality of life through the enhancement of their activities of daily living. The program provides supervised supportive care for frail elderly persons to meet the needs of the functionally impaired individual. Youth, ages 13-23, are encouraged to hang out in YPIs Drop-In Center, participate in activities and programming, or use other services such as tutoring or support services.
