The Senior and Disabilities Services at United Way, also known as the POINT, is a statewide resource network that provides information, referrals, and long-term care options to seniors, adults with disabilities, and their caregivers. Get valuable information and assistance about:

• Information and assistance regarding home and community care
• State Health Insurance Program (SHIP)
• Public assistance programs such as Medicare, Medicaid, SNAP, LIHEAP, and others.
• Information regarding other valuable resources in the community such as healthcare fraud and home care

Programs that are designed to increase public awareness of the measures that people can take to protect homes, businesses and property from the threat of fire. Fire prevention programs provide information about faulty wiring and other electrical hazards, kitchen fires, smoking risks, child-related fire safety, flammable materials storage, brush clearance, fire retardant building materials and ground cover, techniques for fighting a fire until the fire agency arrives (first aid firefighting) and other similar topics. Included are programs that inspect the homes of individual citizens and give them a report which recommends ways to protect their property as well as those that deal with fire safety in a more general way.

School-based one-to-one mentoring programs. Mentoring is done on school grounds, during the school day, throughout the school year.

Services: Extensive values-oriented family, youth and adult programs. Social, aquatic, fitness and physical education activities for all ages. Licensed school age child care, day camping. Special programs for teens, senior adults, the disadvantaged and the handicapped.

Because issues associated with serious disabilities can be physical as well as emotional, SNERC sponsors support groups for patients who have had a brain injury, stroke or spinal cord injury as well as host a family education and orientation group. Stroke Support Group The Fatima All S.T.A.R.S. (Stroke, Together, Advocacy, Recovery, Support) invite you to join "HOPE after STROKE" meetings which take place in the rear of the 4 South dining room at Fatima Hospital. Spinal Cord Support Group Meetings take place in the outpatient therapy gym at Southern New England Rehabilitation Center’s Providence location at St. Joseph Health Center, 21 Peace Street. The gym is located on the third floor on the west wing.

Assisted Daily Living, Inc. provides clients with a wide array of services designed to ensure superior patient care in the familiar comfort of home, often providing a happier and more affordable alternative to nursing, or assisted living facilities. Services can be arranged for a few hours a week or around the clock. ADL begins by reviewing the specific needs of the client through discussions with family members, as well as the care recipient's doctor, social workers or therapists. A customized program of care is developed to manage the client's daily needs. ADL will match the clients needs to the best possible caregiver, based on both professional experience and on ability characteristics. Basic Health Monitoring: Skin - monitor for signs of breakdown, apply pressure reduction measures; Nutrition - monitor weight, nutritional intake, adhere to dietary restrictions; Vital Signs - take blood pressure, temperatures, pulses and respirations. Report abnormalities to physician; Intake / Output - encourage adequate fluids, monitor fluid intake and urine output; Lab Monitoring - perform prescribed blood work and report results to physician; Diet - prepare special prescribed diets, encourage compliance with restrictions; Glucose Monitoring - check blood glucose per orders, report results to physician; Insulin - teach administration of insulin per prescribed regime, follow-up and report compliance to physician. Diabetes: New Diabetic - teach special skin care, insulin injection technique, blood glucose monitoring, hypo/hyperglycemia and related emergency protocol. Medication Management: When capable - remind to take medications and follow-up; When Forgetful - give oral medications prepared by responsible party and follow-up; Supplies - manage supply of prescriptions to have all meds on-hand; Teaching - instruct new medications or monitor changes in regime per plan of care, assess response, side effects and adverse reactions. Catheters (Foley Indwelling and External): New Catheter - teach drainage bag changes and cleaning, securing tubing, trouble shooting dislodged catheters placement, blockages, drainage amounts; Ostomies - Colostomy, Urostomy Changes - insert catheter per authorized plan of care and as needed; New Ostomy - teach ostomy care, preventive skin care, troubleshooting; Care - assist with ostomy changes, monitoring peri-stomal skin. Wound Care: Wound / Bedsore - assess wound, incision or bedsore healing, signs and symptoms of infection and evaluate response to treatment; Teach Care - instruct wound or bedsore regime, evaluate technique, compliance; Treatment - perform wound / bedsore care per prescribed regime, report signs and symptoms infection and progress to physician. Private Duty Nurses: Available to care for patient's in Hospital's & Nursing Homes. Home Assessment: evaluate home safety needs, arrange for adaptive equipment. Therapy: assess, plan, perform and evaluate physical therapy to regain strength and maximal activities of daily living plan. Teaching: instruct prescribe exercise regime and safe ambulation with/without assistive devices. Assistance: help perform prescribed exercises, report progress to physician. Services: Private Duty, Nursing Assistant's available for patients in the Hospitals or Nursing Homes.

Knotty Oak Village is subsidized housing 62+, handicapped or disabled, consisting of 75 units in 13 residential structures. 22 units are located on the second floor.

Hospital charity care requirements cover those essential services necessary to diagnose, correct, or cure a medical condition that endangers life, causes pain and suffering, results in illness or infirmity, or causes a physical deformity or malfunction. Both inpatient and outpatient services are included. To receive full charity care, patients annual family income must be up to and including 200% of the Federal Poverty Level. For partial charity care, patients annual family income must be between 200% and 300% of the Federal Poverty Level may receive assistance based on a sliding scale. In addition to income, hospitals have the option to consider other assets. Also must be denied from Medicaid first depending on income. Patients who get accepted will receive financial aid for a year. **For patients who feel that an application or appeal was wrongfully denied should call the RI Dept of Health info line 222-5960. Our Family Support Center provides basic needs assistance to low income households and connects families to financial literacy and other programs to strengthen their financial condition. The Family Support Center serves low and moderate income Woonsocket residents depending on eligibility requirements for fund-specific programs.

Programs that provide immediate assistance for people who are concerned about their own gambling or the gambling of a friend or family member which may include short-term emotional support, an opportunity to ask questions and discuss alternatives and referrals to confidential treatment services. Helpline staff are generally available via telephone, email, chat and/or text.

General Outpatient Programs are for individuals interested in treatment for Substance Use Disorder symptoms. General Outpatient programs utilize scheduled individual and group counseling sessions and drug/alcohol testing to assist patients in identifying and maintaining healthy life choices Intensive Outpatient Program The Intensive Outpatient Program (IOP) at CODAC is an ideal opportunity for those individuals who find they have a need for increased access to support and treatment. IOP is approximately 8 weeks, and consists of individual and group sessions totaling a minimum 9 hours per week. IOPs may be utilized at any point along the treatment or recovery cycle and based on level of therapeutic need. If you are interested in this level of care speak with your CODAC provider. General Outpatient Services General Outpatient Programs are for individuals interested in treatment for Substance Use Disorder symptoms. General Outpatient programs utilize scheduled individual and group counseling sessions and drug/alcohol testing to assist patients in identifying and maintaining healthy life choices. CODAC counselors also work with the patient to integrate recovery support services in their treatment. We will to continue working with patients who have met their initial goals and want to maintain and strengthen their recovery. CODAC counselors work closely with their patients to achieve individual goals and/or meet the requirements of the referring authority. Most importantly, our programs are designed to assist individuals to develop and integrate a recovery oriented lifestyle that includes family, friends, health, wellness, joy and productivity. All CODAC programs emphasize the value recovery oriented lifestyle benefits and encouraged strongly encourage the integration of family members into the treatment and recovery process.

Our Independent Living/Deaf Services Coordinator works one-on-one with consumers providing assistance in a variety of ways including: They teach consumers to advocate for themselves with tasks such as getting a sign language interpreter for appointments at doctor’s offices, hospitals, Social Security Offices, DHS offices, landlord’s offices etc; accessing community services, requesting reasonable accommodations and addressing general communication issues; provides services to obtain housing. Also provides assistance with bill/letter translation.
