CharterCARE visiting nurse service is unique in that the clinical staff practices primary nursing. They will systematically assess, plan, deliver, and continually evaluate an individualized plan of care for each patient. Skilled Care: Clinical staff provides skilled training in the use, care and management of various medical devices including; G-tubes, J-tubes and foley catheters. We provide training in and assistance with injection therapy, oxygen therapy, ostomy care, tracheostomy care, the administration of insulin for diabetics, medication management, disease management and wound care. Advance Wound Care Every one of our nurses has been trained and is proficient in performing negative pressure wound therapy, a state of the art technology proven to significantly reduce healing time for chronic wounds. (ie: Wound Vac, Renosys and Engenex). Telemonitoring Our telemonitoring program is an adjunct to nursing visits and physician care. Upon the physician order, equipment is installed in the home for the patient. The equipment allows the patient to take daily measurements of weight, blood pressure, pulse and oxygen saturation. Results are monitored daily by an RN. Nutritional Services CharterCARE Home Health utilizes registered dieticians (RDs), food and nutrition experts who have met American Dietetic Association (ADA) criteria to earn the RD credential. RDs are available to assess a patient's nutritional needs, develop and implement nutrition programs, and evaluate and report the results.

We'll help you and your family build meaningful connections to improve your quality of life. Our counseling is confidential. Counseling services- Couples and family counseling- We offer couples and family counseling to support you as you work toward meeting your goals. The Providence Vet Center has a Licensed Marriage and Family Therapist (LMFT) onsite to offer family and couples counseling. Grief and bereavement counseling- If someone close to you has died or you're adjusting to a difficult life change, we're here to support you. The Providence Vet Center offers bereavement counseling in an individual or group setting by a trained bereavement counselor. Mental Health Care- If you're struggling with issues like PTSD, depression, grief, anger, or trauma, we offer counseling and other support. The Providence Vet Center offers individual and group counseling using evidence based therapies such as: Cognitive Processing Therapy (CPT), Cognitive Behavioral Therapy (CBT), Trauma Recovery and Empowerment, Sobriety Support, Solution Focused Therapy, Crisis Intervention, Psychoeducation, Psychosocial Therapy, Prolonged Exposure Therapy, Mindfulness Based Cognitive Therapy, Moral Therapy, Seeking Safety Therapy. Military sexual trauma care- If you experienced sexual assault or harassment during military service, we can help you get the counseling you need. Any Veteran or service member, including members of the National Guard and Reserve forces, who experienced military sexual trauma is eligible to receive counseling. This applies to people of all genders from any service era. The Providence Vet Center offers individual and group counseling by counselors, both male and female, with specific Military Sexual Trauma (MST) training., Minority Veteran Care-If you're a Veteran from a minority community, we can help you find counseling, support, and referral services to meet your specific needs. PTSD Care- If you have symptoms of PTSD after a traumatic event, we can help. We offer assessment and support through private counseling and group therapy. We can also refer you to VA or community counseling for treatment and therapy resources. At the Providence Vet Center we offer individual and group counseling for our Veterans and service members. Our groups run several times a week and at different times of the day. Many of our counselors are even Veterans themselves. Returning Service member care- If you're returning from military service, we'll help you transition to civilian life. We can connect you with educational and career counseling, mental health services, and other programs and benefits that will support your transition. Whole health- Whole health puts you- instead of your illnesses or conditions- at the center of care. We'll work with you to develop a personalized health plan based on your values, needs and goals. The Providence Vet Center offers therapeutic activities such as Yoga, Mindfulness, Fishing Charters, Surfing, Art Expression. Women Veteran Care- If you're a woman Veteran transitioning to civilian life, we can help you find counseling, support, and referral services to meet your specific needs. The Providence Vet Center offers individual and group counseling. Referral Services- Addiction and subtance use care- We can help you overcome substance use problems, from unhealthy alcohol use to life-threatening addiction. We'll connect you to services for your specific needs. The Providence Vet Center has specially trained counselors on staff to support your needs and can facilitate inpatient treatment if necessary. We also provide referral to VA and local community counseling resources. Suicide prevention-Call our Veterans Crisis Line at 988 and press 1 to get support anytime day or night. This service is private, free, and available 24/7. Our Vet Center can also connect you with ongoing counseling and services. Veteran Connections-We can help connect you to VA and community resources that will support you in achieving your goals. The Providence Vet Center may help you know where to go and how to register for VA medical benefits and register for care, filing a military-related disability claim and other forms, understanding your VA education benefits, Housing and home loans, education and referral for VA burial benefits. Community Engagement- We provide private organizations and community agencies education on our Veteran Community and military culture. Together, we build referral networks to expand support for Veterans, service members, and their families. Funds are available to properties built before 1978, properties with pregnant women and children under the age of six, and home-based daycares or foster homes that may contain lead paint. There are no out of pocket expenses or monthly payments. Funds are grants/loans and loans are forgiven after 5 years for owner-occupied properties and 10 years for rental properties. Improvements may include new windows and doors, interior and exterior painting and lead safe yards and play areas.

Provides demonstrations, training, and long-term loan and device reutilization services of specialized telephone equipment to qualified individuals who are Deaf, Hard of Hearing, have a speech disability, or have neuromuscular damage or disease, which hinders them from using a standard telephone. Assistive Technology Many people with disabilities face barriers that prevent them from living independently in their homes, and in many cases, they are also unaware of the many devices that can improve their daily lives. OSCIL works with individuals to acquire appropriate devices and provide training on the use of those devices to enhance the individual’s independence within their home and community. Assistive Technology Access Partnership (ATAP) OSCIL is one of 4 partners participating in the Assistive Technology Access Partnership (ATAP) program. ATAP is designed as a statewide partnership of organizations, each with a targeted assistive technology focus, working together to improve access to and acquisition of assistive technology for individuals with disabilities. Some programs may have income limits. An OSCIL Independent Living Coordinator will be happy to guide you and recommend the best program for your unique needs. OSCIL CAN PROVIDE YOU WITH THE SMART HOME TECHNOLOGY TO MODIFY YOUR HOME TO HELP YOU REACH YOUR GOALS AND INDEPENDENTLY CONTROL YOUR ENVIRONMENT. Individuals with disabilities may apply for technology through this program.
