PASS provides families with the opportunity to take the lead role in directing the services received by their child. The family-centered approach empowers families and individuals to foster independence, support family care giving and decision making, respect family choices and values, and build on individual strengths. Respite Program: a consumer-driven program designed to enhance the quality of life for children with special needs and their families. The provision of Respite enables families to receive support in the delivery of care for their children. Children’s Home-Based Therapeutic Services (HBTS): clinically directed, specialized health services that are designed to meet the individual needs of children diagnosed with moderate to severe physical, developmental, behavioral or emotional conditions. Addresses behavioral concerns and the development of functional, social and communication skills. The HBTS Team is comprised of the parent(s), a clinical supervisor, a home based therapist, a family service coordinator, and when indicated specialty (treatment consultants, speech therapy, physical therapy, and/or occupational therapy). Together, the HBTS Team develops an individualized treatment plan with measurable goals and objectives. Additionally, clinical support and training is provided on a continuous basis to the home based therapist and family.

First Connections at Visiting Nurse Home & Hospice provides free, voluntary, confidential home visits in to pregnant women and families with young children. Conduct home visits: Trained nurses, social workers, and community health workers meet with families in their homes to talk about their needs, answer questions they may have, and conduct comprehensive assessments. If needed, home visitors can make arrangements for interpretation services during home visits. If you are an expecting parent, you may be contacted after the birth of your child to arrange a home visit. Engage families who may benefit from home visits. Link families with medical homes and social services: Home visits are based on the needs of the family. Visit topics may include health education and connections with appropriate healthcare services, human services, and community resources. If a child does not have a medical home, VNHH can help the family find one. Visiting Nurse Home & Hospice can also conduct child wellness screenings and communicate the results with the child’s healthcare provider. Blackstone Valley Community health Care (BVCHC) has established a program that reduces fees charged to patients based on their income and family size. The categories used are based on percentages of “Poverty Level” as annually determined by the federal government. Qualifying patients will pay a nominal fee if they qualify for 100% of poverty. Patients between 101% and 200% will pay an amount depending on their family size and percentage of poverty. Every program has requirements and BVCHC’s Sliding Fee Program is no different.

Programs that offer any of a variety of therapeutic approaches which are intended to maximize the existing cognitive functioning of people who have Alzheimer's disease, Pick's disease or other forms of cognitive impairment. Interventions include structured activities and exercises that are designed for people who have a short attention span and require extra verbal and visual stimulation with the objective of enhancing the individual's perception of the environment, promoting trust, reducing anxiety, avoiding overstimulation and maximizing communication skills.

Our nursing services provide each individual with a personal nurse who'll conduct a comprehensive evaluation of the client's needs and create a care plan that allows them to live as comfortable as possible at home.
