Child Outreach is a free service offered to all children 3 to 5 years old. Child Outreach provides information about your child’s development and resources that are available within your community. It can also help identify children who may need further assessment, intervention and/ or services at an early age to promote positive outcomes in kindergarten and beyond.

Village House offers the comforts of home for those who need short-term recuperation or rehabilitation following a hospital stay. They also offer long-term care for those with chronic conditions or respite care when family caregivers need someone to step into their shoes for a short time. For those with a loved one with Alzheimer’s disease or related dementia, the special care rooms provide the peace of mind that only a secure environment staffed by those specially trained in Alzheimer’s and dementia care can provide. The SSI State Supplemental Payment (SSP) provides a small supplement to recipients of federal Supplemental Security Income (SSI) and some non-SSI recipients who are low-income personas with disabilities and older adults with limited resources to pay their living expenses. The federal Social Security Administration (SSA) will continue to administer the SSP for individuals residing in residential care or assisted living. DHS operates in-house insurance of the SSP for all other eligible individuals.

The Ocean State Center for Independent Living (OSCIL) works in collaboration with the Gift of Hearing Program at the University of Rhode Island (URI) Speech & Hearing Center to provide low cost hearing aids to those in need.

Lincoln Village located in Lincoln Rhode Island is a LIHTC development offering one, two, three, and four bedroom apartments. Designed for singles and families, income guidelines are established through the Tax Credit program.

Services: Houses 29 families and 8 women daily. Shelter stays are limited to 90 days but can be renewed based on progress demonstrated by participants or impending permanent housing. Case managers assist participants with healthcare, employment and establishing permanent housing. Help also available for: mental health issues, addiction, developmental delays in children, parenting and domestic violence. Case managers access services directly through Family Resources Community Action Program or collaborative agreements with other local non profit organizations.

The substance abuse task force identifies and addressed the issue of substance abuse in the city and its mission is to develop, implement, and advocate for effective community-based prevention initiatives.

Women, Infants and Children (WIC) Supplemental Feeding and Nutrition Program provides food vouchers, allowing them to purchase milk, eggs, cheese, juice and iron fortified cereals and formula. The WIC coordinator provides nutrition education and counseling to all program participants on a monthly basis.

A program of BVCAP: RSVP provides opportunities for seniors 55+ to share their time, experience, knowledge and interests with others in the community. RSVP volunteers are connected to networks of seniors who use their skills to provide services and support throughout the community. RSVP volunteers:

• Provide administrative support, nutrition education, SNAP resources, and community referrals for hunger
• Deliver meals to home bound clients 5 days a week
• Provide home visits and companionship to homebound clients
• Participate with the Wellness Initiatives exercise trainings such as armchair aerobics, water aerobics, line dancing, and tap dancing at Senior Services in Woonsocket
• Assist in ongoing recruitment, enrollment, and program management of A Matter of Balance (an Evidence Based Fall Prevention Program)
• Collect, sort, recycle, reuse, or repurpose recyclable materials in Woonsocket RI and surrounding areas
• Provide education, outreach, and information on disaster preparedness
• Participate in recognizing and assisting veterans and their families with clothing, heating assistance, weatherization benefits, nutritional information, SNAP, and other community and government resources
• Partner with persons of disabilities from one on one or small group activities in the catchment area

Thundermist runs school-based health centers to provide primary care services to students in schools, during the school day. Services at The Health Hut include: physical exams (including school and sports physicals), immunizations, sick visits, treatment of minor injuries, health education, chronic care management (i.e. asthma, diabetes), and dental services-including examinations and hygiene services. School-based health centers work closely with school personnel including principals, school nurse teachers, and student support staff. They do not take the place, or do the job of, the school nurse teacher. School-based health centers are helpful to parents because: It is often difficult to get time off of work to take a child to the doctor. Many families face transportation, language, or financial barriers in traditional health care settings. Adolescents often do not seek out health care because of embarrassment. Adolescence is an important time to have access to a trusted health provider.

Emergency Shelter: 4 bedrooms, for 9 to 11 people. One handicapped accessible room. Services available: referrals to educational, vocational, and employment resources, individual meetings with advocates to establish goals for victims and their children; Weekly support groups both in-house and community based; HIV testing. Residents are responsible for their own family's food:kitchen and storage space is provided. Food assistance available if needed. Important Policies: Location of shelter is confidential, residents are expected to maintain confidentiality: Maximum of 2 nights out/month, except in emergencies; No time limit as long as working on goals of permanent housing and following all shelter rules; Residents are responsible for their own medications, lockers are available; residents will have to lock up medications. Smoking allowed in designated outdoor areas; A harm reduction model is used-sobriety is not required, however residents are expected to respect peers in recovery; Bedrooms and common areas are shared; There may be a possibility teenage boys are housed ***Financial assistance, including rent and utilities, to victims and survivors of domestic violence, sexual assault and human trafficking who are fleeing abusive situations, at risk of becoming homeless or at risk of returning to their abuser as a result of COVID-19. Supportive services will also be provided, including expanded hotline hours, more clinical support, additional advocacy services and assistance with basic needs such as food and transportation.

The Northern Rhode Island Collaborative (NRIC) is a non-profit educational service center committed to providing special education programs and educational services to partner school districts and community partners throughout northern Rhode Island. Foundations Academy (formerly the Regional Alternative Program): Provides special needs students - diagnosed with behavioral and emotional disorders - with an alternative to traditional schools. Foundations Academy (formerly the Specialized Alternative Program): Students who learn differently and have different abilities need the help and guidance of educators who are specially trained to develop and maximize those abilities. Educators in the Foundations Academy work to identify a student’s skills and their potential for learning new skills. Educators, parents and students then develop a progressive plan tailored to an individual’s educational needs and social well-being.

Established in 1966 in historic Warren, Rhode Island, Grace Barker Health is a family-owned healthcare provider of skilled nursing, short-term rehabilitation, assisted living and adult day health services. Administrators/Owners Mark and Mary Beth Lescault are Warren natives whose family has operated the facility since its founding in 1966 by Mary Beth’s mother, Linda Machado Grace Barker Health is a nursing home and assisted living community that provides long term care, short term rehabilitation, and adult day care services. Assisted living for those who want to maintain independence, but need assistance with daily tasks including assistance with housekeeping, bathing, and dressing.

Adult Day Health Care Program is for adults with chronic health conditions and medical or physical limitations. Provides a combination of social activities, health care and rehabilitation services, and personal care. Participants attend on a set schedule from two to five days a week based on availability.
