Adult day program for the elderly and disabled that offers a six week health education program called Take Control of Your Health. Nutritional meal provided at noon and two daily snacks, senior advocacy program, daily fitness program, spiritual, music, games. RIDE program for transportation, and a social worker on staff. Individuals with diabetes, high blood pressure, depression, memory loss are especially encouraged to attend the Center because their program is specifically designed for these health conditions. Their staff is composed of CNA's, and they help with care management. They also monitor blood pressure and blood sugar levels for individuals. They take donations.

Disability Determination Services (DDS) is a unit within the Office of Rehabilitation Services. DDS is the state agency which determines the medical eligibility of Rhode Island residents (children and adults) who have applied for cash benefits under the disability programs administered by the Social Security Administration (SSA). The two SSA disability programs are Social Security Disability Insurance (SSDI) (Title II) and Supplementary Security Income (SSI) (Title XVI). The DDS is responsible for collecting medical evidence of record and determining whether the claimant is disabled and/or blind as defined by the Social Security Act. Medical, psychological, and audiological examinations and tests are used to better understand the disability and needs for specific types of services.
A collaborative group of local comprehensive and specialized information and referral providers who have agreed to coordinate information and referral service delivery to provide seamless access to information about community resources via the three digit number assigned for this purpose, 211. Depending on the size and nature of the community, 211 systems may consist of only a few organizations that provide information and referral or may include a diverse array of organizations that furnish some type of I&R. Some of these organizations may supply a very specialized type of I&R service (e.g., for people with disabilities or older adults), while others may provide a comprehensive range of assistance. The coordination of all I&R services and programs within a community and, where relevant, beyond, constitutes the 211 system.
