An emphasis is placed on residents succeeding, dignity, and support of family members. Short Term Rehabilitative Care Short Term Rehabilitative Care is designed for individuals in need of rehabilitation following hip or knee replacement, heart attack, stroke, or other acute medical event. Our goal is to rehabilitate individuals to restore their previous level of functioning with the goal of returning back to their home. Skilled nursing services; on-site physical, occupational, and speech therapies are available. Short term stays range from one week to several weeks, depending upon the individual’s medical condition. Hospice Suite The Hospice Suite provides care for terminally ill individuals who qualify for general inpatient level of care. The Suite affords residents a lovely homelike environment with ample room for visiting family members.

Pediatrician

The UCP Rhode Island Children & Family Services program provides services to children with disabilities and their families under Rhode Island�s Home Based Therapeutic Services Program (HBTS). Medicaid eligible children (up to age 21) are provided comprehensive periodic evaluations to supplement the treatment of various medical conditions. Works with families to develop service plans for their children and assists the families to secure HBTS funding. Once a plan is approved, UCPRI provides case management and services, therapies, and training for the child and family. The object is to apply individualized services that permit the child to live at home and take an active part in school and community. UCPRI�s services may help relieve some of the financial strains that make raising a child with disabilities so stressful to both the family and child. Other Children & Family Services:

• CAP (Community Activities Project) provides financial support enabling children with disabilities to participate in community and recreational activities.

Provides information, referral and advocacy to families with children who have special needs. UCPRI will assist families in identifying their needs (home health care, learning, recreation) and work with them to secure funding. Adult Education: UCP Rhode Island�s Adult Education Community Classrooms meet throughout RI. Instruction is individualized, based on the personal learning goals of each student. Most classes meet for 3 hours a week for 12 weeks. A multi-purpose senior center. Programs supported by our senior centers are quality meals, health seminars, socialization, exercise programs, reliable transportation, and other engaging activities.

For adolescent girls, 13-17. Provides individual, group and family counseling in a long-term residential treatment program for women in recovery from chemical dependency. Daily living in a family atmosphere develops a sense of normalcy and belonging in a non-threatening, supportive, educative and chemically free environment. The program places a strong emphasis on the Twelve Step Program of Alcoholics Anonymous and Narcotics Anonymous. Assertiveness training as well as physical and recreational activities are designed to improve health and appearance. Special emphasis is placed on helping each woman develop positive feelings of self-worth that will support the ultimate goal of sobriety. A relapse prevention specialist provides residential and outpatient relapse prevention services. Eastman House also offers a continuing after care program. Cedar Crest Subacute and Rehabilitation programs, a 156 bed medical center, were designed for patients with physical, medical and cognitive impairments who would benefit from patient-centered care and an interdisciplinary team approach. Our goal is to work with patients to achieve their optimum level of functioning. When those treatment goals are met, the patient is ready to move on. Treatment plans can include physical therapy, occupational therapy and speech-language therapy, along with nursing care oriented to the needs of rehabilitation patients.
