The RISE Clinic at The Miriam Hospital provides specialty consultation related to tuberculosis (TB) care in partnership with the Rhode Island Department of Health. Assisted Daily Living, Inc. provides clients with a wide array of services designed to ensure superior patient care in the familiar comfort of home, often providing a happier and more affordable alternative to nursing, or assisted living facilities. Services can be arranged for a few hours a week or around the clock. ADL begins by reviewing the specific needs of the client through discussions with family members, as well as the care recipient's doctor, social workers or therapists. A customized program of care is developed to manage the client's daily needs. ADL will match the clients needs to the best possible caregiver, based on both professional experience and on ability characteristics. Basic Health Monitoring: Skin - monitor for signs of breakdown, apply pressure reduction measures; Nutrition - monitor weight, nutritional intake, adhere to dietary restrictions; Vital Signs - take blood pressure, temperatures, pulses and respirations. Report abnormalities to physician; Intake / Output - encourage adequate fluids, monitor fluid intake and urine output; Lab Monitoring - perform prescribed blood work and report results to physician; Diet - prepare special prescribed diets, encourage compliance with restrictions; Glucose Monitoring - check blood glucose per orders, report results to physician; Insulin - teach administration of insulin per prescribed regime, follow-up and report compliance to physician. Diabetes: New Diabetic - teach special skin care, insulin injection technique, blood glucose monitoring, hypo/hyperglycemia and related emergency protocol. Medication Management: When capable - remind to take medications and follow-up; When Forgetful - give oral medications prepared by responsible party and follow-up; Supplies - manage supply of prescriptions to have all meds on-hand; Teaching - instruct new medications or monitor changes in regime per plan of care, assess response, side effects and adverse reactions. Catheters (Foley Indwelling and External): New Catheter - teach drainage bag changes and cleaning, securing tubing, trouble shooting dislodged catheters placement, blockages, drainage amounts; Ostomies - Colostomy, Urostomy Changes - insert catheter per authorized plan of care and as needed; New Ostomy - teach ostomy care, preventive skin care, troubleshooting; Care - assist with ostomy changes, monitoring peri-stomal skin. Wound Care: Wound / Bedsore - assess wound, incision or bedsore healing, signs and symptoms of infection and evaluate response to treatment; Teach Care - instruct wound or bedsore regime, evaluate technique, compliance; Treatment - perform wound / bedsore care per prescribed regime, report signs and symptoms infection and progress to physician. Private Duty Nurses: Available to care for patient's in Hospital's & Nursing Homes. THERAPIST'S - PT, OT, ST: Therapists including physical therapists, occupational and speech therapists assist in the rehabilitation of those with physical injuries or disease. Restoration of mobility, strength, dexterity, balance and communication skills is often the purpose for therapy. Helping individuals who are disabled by physical injuries or disease to regain maximum function with activities of daily living is always a main priority. Home Assessment: evaluate home safety needs, arrange for adaptive equipment. Therapy: assess, plan, perform and evaluate physical therapy to regain strength and maximal activities of daily living plan. Teaching: instruct prescribe exercise regime and safe ambulation with/without assistive devices. Assistance: help perform prescribed exercises, report progress to physician. MSW: Social Workers assist in the evaluation of social, emotional and environment factors affecting the ill and disabled. They may provide family or individual counseling for those in need or a crisis situation. Medical Social Workers offer support and often help indentify and locate appropriate community resources: Home care services, adult daycare, respite, long term care planning, financial assistance and support groups. Nursing Assistant's / Home Health Aides: Eating - Plan and provide nutritious meals, adequate fluids. Assist with feeding; Bathing - Assist with personal care: bathing, transferring in/out tub/shower, bed-baths as needed; Dressing / Grooming - Assist with dressing and glasses, hearing aides and other special orthotics (i.e. braces), hair care, shaving, oral hygiene; Ambulation - Assist with walking, safe use of assistive devices, range of motion and strengthening exercises; Toileting - Assist to bathroom, bedside commode, urinal, bedpan usage; Incontinence - assist changing diapers, undergarments, pads and perform related skin care. Programs that pick up and impound or return to their owners domestic animals who have strayed from their homes. Also included are programs that enforce leash laws and animal sanitation ordinances and those that handle complaints regarding domestic animals who have bitten someone or are otherwise making nuisances of themselves. Most animal control programs are unable to handle nuisance wildlife unless the animal is sick or shows symptoms of rabies.

Thrive’s Community Support Program (CSP) is a coordinated system of care designed for adults aged 18 and older who are struggling with a severe and persistent mental illness and require a higher level of care. CSP participants have received a qualifying diagnosis of a psychotic disorder, mood disorder, obsessive compulsive disorder and have had difficulties engaging with treatment.This program enables individuals to maintain psychiatric stability, increase their quality life in the community, and ultimately achieve their own personal recovery. Qualifying clients will have access to psychiatric services, counseling, case management services, education, peer support, residential services, and transportation to and from medical appointments. Thrive's Community Support Program is comprised of three integrated support teams including the Assertive Community Treatment (ACT) Team and Integrated Health Home (IHH) Teams. An Outpatient level of care is available to those who have similar needs but require less intensive care.

Provides short-term counseling for individuals, couples and families; information and referral about on-base and community service agencies; employment assistance, jobs skills bank for dependents; education programs in such areas as parenting, stress management, personal financial management; consumer information.

Hospital charity care requirements cover those essential services necessary to diagnose, correct, or cure a medical condition that endangers life, causes pain and suffering, results in illness or infirmity, or causes a physical deformity or malfunction. Both inpatient and outpatient services are included. To receive full charity care, patients annual family income must be up to and including 200% of the Federal Poverty Level. For partial charity care, patients annual family income must be between 200% and 300% of the Federal Poverty Level may receive assistance based on a sliding scale. In addition to income, hospitals have the option to consider other assets. Also must be denied from Medicaid first depending on income.
