The East Bay Senior Medicare Patrol (SMP) Program helps Medicare and Medicaid beneficiaries prevent, detect, and report health care fraud. SMP not only protects seniors, it also helps to preserve the integrity of the Medicare and Medicaid programs. Senior Medicare Patrol staff and volunteers work with beneficiaries to: -Review their Medicare Summary Notices for accuracy -Protect their identity -Identify and report errors on their health care bills -Identify deceptive health care practices, such as illegal marketing, providing unnecessary or inappropriate services, and charging for services that were not provided -SMP also conducts informational sessions for seniors about avoiding Medicare and Medicaid scams.

Assisted Daily Living, Inc. provides clients with a wide array of services designed to ensure superior patient care in the familiar comfort of home, often providing a happier and more affordable alternative to nursing, or assisted living facilities. Services can be arranged for a few hours a week or around the clock. ADL begins by reviewing the specific needs of the client through discussions with family members, as well as the care recipient's doctor, social workers or therapists. A customized program of care is developed to manage the client's daily needs. ADL will match the clients needs to the best possible caregiver, based on both professional experience and on ability characteristics. Basic Health Monitoring: Skin - monitor for signs of breakdown, apply pressure reduction measures; Nutrition - monitor weight, nutritional intake, adhere to dietary restrictions; Vital Signs - take blood pressure, temperatures, pulses and respirations. Report abnormalities to physician; Intake / Output - encourage adequate fluids, monitor fluid intake and urine output; Lab Monitoring - perform prescribed blood work and report results to physician; Diet - prepare special prescribed diets, encourage compliance with restrictions; Glucose Monitoring - check blood glucose per orders, report results to physician; Insulin - teach administration of insulin per prescribed regime, follow-up and report compliance to physician. Diabetes: New Diabetic - teach special skin care, insulin injection technique, blood glucose monitoring, hypo/hyperglycemia and related emergency protocol. Medication Management: When capable - remind to take medications and follow-up; When Forgetful - give oral medications prepared by responsible party and follow-up; Supplies - manage supply of prescriptions to have all meds on-hand; Teaching - instruct new medications or monitor changes in regime per plan of care, assess response, side effects and adverse reactions. Catheters (Foley Indwelling and External): New Catheter - teach drainage bag changes and cleaning, securing tubing, trouble shooting dislodged catheters placement, blockages, drainage amounts; Ostomies - Colostomy, Urostomy Changes - insert catheter per authorized plan of care and as needed; New Ostomy - teach ostomy care, preventive skin care, troubleshooting; Care - assist with ostomy changes, monitoring peri-stomal skin. Wound Care: Wound / Bedsore - assess wound, incision or bedsore healing, signs and symptoms of infection and evaluate response to treatment; Teach Care - instruct wound or bedsore regime, evaluate technique, compliance; Treatment - perform wound / bedsore care per prescribed regime, report signs and symptoms infection and progress to physician. Private Duty Nurses: Available to care for patient's in Hospital's & Nursing Homes. THERAPIST'S - PT, OT, ST: Therapists including physical therapists, occupational and speech therapists assist in the rehabilitation of those with physical injuries or disease. Restoration of mobility, strength, dexterity, balance and communication skills is often the purpose for therapy. Helping individuals who are disabled by physical injuries or disease to regain maximum function with activities of daily living is always a main priority. Home Assessment: evaluate home safety needs, arrange for adaptive equipment. Therapy: assess, plan, perform and evaluate physical therapy to regain strength and maximal activities of daily living plan. Teaching: instruct prescribe exercise regime and safe ambulation with/without assistive devices. Assistance: help perform prescribed exercises, report progress to physician. MSW: Social Workers assist in the evaluation of social, emotional and environment factors affecting the ill and disabled. They may provide family or individual counseling for those in need or a crisis situation. Medical Social Workers offer support and often help indentify and locate appropriate community resources: Home care services, adult daycare, respite, long term care planning, financial assistance and support groups. Nursing Assistant's / Home Health Aides: Eating - Plan and provide nutritious meals, adequate fluids. Assist with feeding; Bathing - Assist with personal care: bathing, transferring in/out tub/shower, bed-baths as needed; Dressing / Grooming - Assist with dressing and glasses, hearing aides and other special orthotics (i.e. braces), hair care, shaving, oral hygiene; Ambulation - Assist with walking, safe use of assistive devices, range of motion and strengthening exercises; Toileting - Assist to bathroom, bedside commode, urinal, bedpan usage; Incontinence - assist changing diapers, undergarments, pads and perform related skin care. Services: Check In: mornings, afternoon or evenings, Wake up and Tuck in. Supervision during the day time or overnight. Senior Inpatient Treatment Unit: Inpatient services for adults age 65 years and older, the geriatric unit is led by a specialized treatment team well versed in conditions such as Alzheimer’s and other cognitive disorders as well as depression, anxiety and other mood or psychotic disorders that may cause behavioral difficulties. The clinical team works to stabilize the patient for return home or to an assisted living campus with a comprehensive transition of care process. Memory and Aging Program: An internationally renowned research program dedicated to developing breakthroughs in prevention and treatments for Alzheimer’s disease, while this is not a clinical care program, it offers hope to those at risk or exhibiting memory loss symptoms. With multiple research trials underway, including registry for future studies, participants are people with normal memory who may be at jeopardy for dementia and those who are already experiencing signs of cognitive decline.
